Trivia: In the Area 51 levels when the player is inside the lab, a women on the intercom is heard saying names such as Lieutenant Deal, Doctor Francis, Captain Santiago, Doctor Lovering and Nurse Robinson. Those names are a reference to the band The Pixies.

Continuity mistake: In the 2nd DataDyne level when Joanna punches the guard in the elevator, the guard is facing away from the elevator door and has his head below the wall. But in the next level, when the guard wakes up, he has his head down below the door, even though he was supposed to be unconscious.

Continuity mistake: In the first DataDyne level, when the player punches Cassandra in her office, the necklace isn't on Cassandra's collar because the player has it. But then in the G5 Building level when the player sees Cassandra in the meeting room via Camspy, the necklace is back on her collar.

Trivia: In the Pelagic II level, the guards are wearing outfits similar to Mario because the game is published by Nintendo.

Continuity mistake: At Didi's baby shower, when Boris tells the babies his goat is saying hello to them, he has nothing in his hand. In the next shot, chocolate coins appear.

Bug: In the Terminus hotel level, when Agent 47 as the player kills the lady with hair rollers and painted face in the private area, the lady is moving while killed but as you leave and then come back near her, she's not moving but starts moving when you shoot her or drag her.

Trivia: In the Chicago level, there is a piece of swiss cheese hidden inside the toilet seat in the bathroom.

Trivia: After the end credits finish, a line appears saying "Perfect Dark is Forever."

Other mistake: In the final level, Mr. Blonde's Revenge, Mr. Blonde is the player, but several guards say "get her" before shooting the player. This mistake also happens when you play any level with Velvet as your buddy, because she says the same thing before shooting the guards.

Continuity mistake: The rocking chair that Lou is sleeping on is facing the side away from the doorway alongside the table next to it. When the delivery guy starts walking in to pick up the box and give the receipt to Lou sleeping, the chair and table are straight next to the doorway.

Continuity mistake: When Grandpa Lou is in the rocking chair taking a nap, he has a newspaper in his hand, and Boris' goat tears a piece of paper off it. When the babies see and get on the Reptar wagon, the newspaper has changed into a blank piece of paper.

Trivia: After the end credits, there is a post-credit scene, possibly taking place the morning after the babies were rescued and back home, with Boris' goat while Grandpa Lou is sleeping in the Reptar wagon. The goat bumps his head into the wagon and the wagon with Grandpa Lou inside napping drives away on the road with the goat following it. Susie's brother is seen waving as the wagon is driving away. Ever wonder what Grandpa Lou's reaction would've been if he woke up and noticed the wagon driving him away from the house? He would've felt possibly scared and got lost like the babies did during the movie.

Continuity mistake: When the monkey's owners are having coffee and donuts at the train station, they notice that the monkeys are on the train. When they go out going after the train, they seem to be on a sidewalk next to the railroad tracks. But then in the next shot when the train fully has ran away, boards have appeared, basically to keep the owners safe from those tracks.

Continuity mistake: When Grandpa Lou and Stu are looking through the front driveway noticing the Reptar wagon and babies are missing when seeing the bottle on the carpet, the bottle is still on the carpet with spilled milk. When they return home looking around the house for the kids, the bottle is gone and the carpet is completely clean.

Continuity mistake: When Stu shows Tommy a picture of him and Drew as younger siblings when he's explaining the brothers' relationship, the picture is still in his hand. But in the next shot it's not.

Continuity mistake: When the babies go visit Dil in the living room, there is a ball and a block behind Dil in front of the couch. When Dil hits Tommy and Chuckie with his rattle, the ball and the block both are gone.

Continuity mistake: When Ed is fixing the Good Shake machine, a stack of cups are straight but in the last shot when Ed is spinning around, the stack of cups are flipped over.

Continuity mistake: When Dexter is working at Mondo Burger before getting fired, slices of tomato disappear and reappear between shots while he is making his first burger with all that mess.

Continuity mistake: When Ed flies out of the window to escape Demented Hills, his hat is gone. But in the next shot when he lands on Dexter on the ground, his hat is back.
