Factual error: Chakotay says "if our orbit starts to decay, Voyager will begin to feel the effects of the differential, and we'll begin aging hundreds of times faster than we would in normal space". Whilst it is true that they would be aging faster relative to normal space, they would not instantly become old. Time would simply slow around them, so whilst they would be aging faster relative to normal space, they would not all of a sudden become really old - which is how it is made out to be. They would all age the same amount whether in a standard orbit or in a more decayed orbit. (00:06:37)

Suggested correction: There is nothing incorrect about what he said. They will start ageing hundreds of times faster than in normal space.
Aging implies getting/feeling older. They'd only be "aging" relative to normal space. What would happen would be more akin to time travel, with the universe getting older around them.
But the point is, they wouldn't age faster just because "normal" time slows down. If they spent a year on the planet, they'd age 1 year, not 100 years.

Corrected entry: Toward the end of the episode when Seven is speaking with the Doctor she references his musical error earlier in the episode. She says he was off by "decibels." Decibel is a unit of volume, not pitch.
Correction: Seven says "your vocal modulations deviated by 0.30 deci-hertz", not decibels. Hertz is a measurement of frequency, or pitch.

Corrected entry: Within the first 15 seconds as the camera pans the crowd before the Tsunkatse match, we see a good 30 or so federation crew members. Impossible seeing as they have not come across this event before, and the only Starfleet crew in the Delta quadrant are on Voyager. (00:00:15 - 00:01:00)
Correction: The federation people you see in the stands are crew members of Voyager, not random Alpha quadrant federation members.
Additionally, if they were regular Starfleet members from the Alpha Quadrant, they'd be wearing the newer, black/grey uniforms with a coloured undershirt, rather than the brighter uniforms still in use on Voyager.

Plot hole: In this episode Kes states that she has to decide now whether to have a child or not because Ocampa women can only get pregnant once and deliver one child. If that was the case they would have died out a long time ago, or never even evolved, as two people only getting one offspring would reduce the population to 50% of the original figure each generation.

Suggested correction: It's also possible that it's simply required of Ocampan women to give birth the first time they go through it, and can then experience it again.
Nothing in the dialogue suggest Ocampa's can have additional children. While we can speculate about fictitious species, it's still a plot hole due to writing. Kes states she's going through the "elogium" which is a time of change where her body prepares for fertilization. She then explicit states the "elogium" only occurs once. While the doctor compares it to puberty, the elogium is both sexual maturity and "heat", that is, the time a female is ready for fertilization.
But it doesn't ever state the normalcy of birthing for Ocompans. Perhaps sextuplets is the norm?
Kes frequently used the word "child." If it was normal to give birth to more than one, she would know this and should say "children."
Do not forget that this is all done through the universal translator. For all you know the Ocompan word for child and children is the same so the translator cannot tell the difference.

Factual error: To protect the warp core, Janeway instructs Paris to move away from the rings at Warp 7. Moments later (Janeway, Torres and Tuvok are still in the same position), Paris tells Janeway they are 0.6 light years away from the rings. Warp 7 is 656 times the speed of light. It would take about 8 hours to travel 0.6 light years at Warp 7.

Suggested correction: The warp scale, especially since it's fictional, has never been precisely defined.
While it's never been defined, certain aspects of warp speed drive and the length of a light year are accepted. Even if Warp 7 is double what is suggested in the mistake, it would still be 4 hours. The scene in question shows no lengthy travel at Warp 7.

Plot hole: Gunfire damages the holodeck controls, and the computer announces that safety protocols are now offline. This implies that they were online before the gunfire. If they were still online before the gunfire, the bullets would not have damaged the controls.
