Continuity mistake: In the first shot of Kerry watching the Jigsaw tape you can see 2 folders to her left. She fast forwards the tape a bit, the shot cuts back to her, and one folder is inside of the other. Also, all the pictures spread out in front of her change positions and placement drastically. Continuity mistake: After the scene of Tony there is a shot of Kerry and Rigg, the detective is in the background inspecting a piece of the chain. The shot moves to the background detective for a second and he looks up at something. The shot instantly cuts back to Kerry and Rigg and the background detective is suddenly looking down at the chain again. (00:10:25)

Continuity mistake: There is a shot when Tony (chained man) is listening to Jigsaw's message where blood drops out of his mouth and runs down his chin, but in the next close-up of Tony you can see that most of the blood has mysteriously disappeared (he still has blood on his chin, but it looks the same way that it did BEFORE he drooled blood all over the place). Continuity mistake: The blood streaks on the chained man's chest continually changes from almost covering his chest, to less noticeable. They probably tried to give the appearance of his continual bleeding, but the shots didn't make it into the movie in sequence. (00:06:25)

Continuity mistake: In the close-up shot of Lynn removing the piece of Jigsaw's skull you can see that she's holding the instrument in her left hand (look at the position of her fingers), but in the farther away shots she has it in her right hand. (further evidenced by later in the scene where you see a close-up and she's still holding the instrument in the left hand and a knife is in her right). (01:04:30)

Continuity mistake: When Kerry reaches up the second time to try and get the key you can see the acid burns (a few, but not a whole lot of blood) on her hand. There is a quick cut where her hand is poised over the jar of acid and her hand has suddenly become almost entirely red and bloody. (00:06:10)

Deliberate mistake: The scenes where the left, then the right foot is being twisted are the same shot, just flipped. You can tell because the blood on the foot and the blood on the machine are in the exact same locations in both shots.

Continuity mistake: The blood smear on Lynn's cheek changes size and shaper during the scenes when Jigsaw is hallucinating and tells her that he loves her.

Continuity mistake: Amanda freaks out and pins Lynn to the wall after Lynn says "He doesn't even know you're there". Amanda's hand is around Lynn's neck, choking her. The screen cuts to Jigsaw very briefly (all he says is, "AMANDA!"), then back to Amanda, now Amanda's hand has moved up to being on Lynn's chin. Not only would moving her hand from Lynn's neck to her chin not serve any purpose, but the screen cuts happened too fast to allow for the hand movement. (01:14:25)

Continuity mistake: Look at Jigsaw's oxygen mask first time Lynn tells him "Look at my flashlight". The mask is next to his neck, lying flat on his shoulder. The shot cuts to Lynn for a second, then back to Jigsaw, now the mask is standing vertical and almost touching his cheek. (00:36:25)

Continuity mistake: The positioning of Amanda's green sheet changes drastically during her cutting scene.

Continuity mistake: When Amanda dies, her left arm is outstretched and her hand lying vertically on it's left side. There is a quick cut to her body during Jigsaw's final message and you can see that her hand is lying flat on the floor, then it cuts to a close-up of her hand and it's back on its side again. (look at her watch for a reference point to her hand position).

Continuity mistake: The red cloth (bag?) that's covering the gun changes its position DRASTICALLY between the first time you see it (then he talks to his daughter when she comes in the room) and then the second time you see it. The first time the back half of it is folded back over itself and it's almost tucked under Jeff's leg. The next time you see it it's completely spread out flat. Jeff didn't rearrange it while he talked to it daughter, it just "magically" happened. (00:33:00 - 00:33:55)

Continuity mistake: The blood spots and streaks on the chained man's body are different in the close up shots of the chains than they are during other shots. (00:06:00)

Continuity mistake: When Amanda tells Lynn "Let me give you the simple version" there are several hairs that are across Lynn's face, the next shot of Lynn has her hair back nice and neat. (00:27:35)

Continuity mistake: The blood spot on Lynn's left sleeve (from when Jigsaw has a seizure) constantly changes size and shape throughout the whole movie. (00:51:40)

Continuity mistake: When Kerry is talking to Rigg and the detective she is almost directly in front of the door. When the other girl says "You won't believe this, we got the tape out clean" the camera pans around and you can see that Kerry is now in front of the chalk boards. (00:11:05)

Continuity mistake: The knot and string position on the tape player is different during the close up of it on "The Rack" guy's chest than it is during the far away shots.

Continuity mistake: The guy that is in chains stands up, looks around and takes in what he sees. There's a shot of him from the front and his hands are semi-clenched in FRONT of him, around waist level. The shot cuts to behind him and his hands are a good 4 or 5 inches OUTSIDE of his hips. Continuity mistake: While Jeff is contemplating burning his son's possessions his hair constantly changes from messed up and down, to pushed up and away from his face. One of the shots you can see Jeff put his hand to his forehead in agony, so this explains that part of the hair change, but the changes happen several times during the same scene.
