Visible crew/equipment: When we see Kim's vision, there is a shot of the car going down the highway. In some shots you can see tyre marks from the camera car on the hard shoulder.

Visible crew/equipment: When walking down the corridor to see Clear, there is a sticker on the door with Ward A written on it. If you watch the sticker the light that is reflected is moving around. Clearly from the camera. (00:31:00)

Visible crew/equipment: Just after Clear is kicked out the ambulance, there is a shot of the kid being dragged out of the way of a speeding van. Look at the van and you will see a boom mic. (01:06:30)
