Other mistake: Noticed in police procedurals but true in various contexts - whenever a group of people spontaneously lay out a case or explain something to someone, it's magically a well-oiled presentation. Person 1 says their bit, then Person 2, then Person 3 takes over the narrative. There's never any interruption or stumbling about which person is going to cover which section.
