Other mistake: Noticed in police procedurals but true in various contexts - whenever a group of people spontaneously lay out a case or explain something to someone, it's magically a well-oiled presentation. Person 1 says their bit, then Person 2, then Person 3 takes over the narrative. There's never any interruption or stumbling about which person is going to cover which section.

Other mistake: People gaining access to a computer system they've never seen or used before, but manage to figure out exactly how to do the thing they need to do, often in a very limited timeframe. No clicking around trying to find the right area or the right command.
