Leon (Bradley Cooper) kills Mahogany (Vinnie Jones). Mahogany's victims are food for creatures that have existed since a long, long time ago and the train takes the bodies to where the creatures live. The train driver kills Maya (Leslie Bibb), takes out her heart, rips off part of Leon's tongue and tells him "Now serve us without question." The cop who talked with Leon and Maya knows about the train and also serves the creatures. Leon becomes the next butcher and it's revealed that he was the killer in the opening scene.
