Continuity mistake: After stabbing the bad guy at the beginning, Callahan for some reason pulls back and somehow dives past a door managing to drag in a grown woman who was all crouched down by the entrance. Wonky physics of it aside, the bad guy shoot at him, but shoots just a hole in the door frame. The hole appears to be CGI, and in fact when Callahan shoots back from the bedroom, the hole is gone. Continuity mistake: Jen unboxes the stuff she grabbed from the basement where she spotted the railroad tracks. She picks up a large vase which she shows to her business partner; the hand at the bottom is the vase is the left or the right one depending on the camera angle. (00:10:40)
Continuity mistake: Michelle Pfeiffer's costume. First we see her toes being painted, drying with tissue between each of her toes. She gets up from the chair to take the peppers out of the oven, talking to Fred about being a writer. She then goes out the door across the street with socks on and Fred's slippers. She never put on any socks or pulled out the tissue.
Continuity mistake: When Ivan invites Richie onto his yacht for the first time, a jet ski pulls into Ivan's close-up, and disappears after it cuts back.
Continuity mistake: During the opening scene of the film we see two boxers fighting in Julian's boxing club. The fighter in red trunks knocks out the one in blue trunks and is announced the winner. Two men then come into the ring and, between shots, we see them pick up the loser off the floor twice. (00:02:51)
Continuity mistake: As El Camaleon pulls the Cuba Gooding Jr. face off, there's an unshaven neck with a tattoo visible under it. After the cut, Lady Gaga's face (also presumably unshaven admittedly, but not visibly so) comes to light under a mask of visibly lighter skin tone. (00:45:05)
Continuity mistake: When Irving goes to see Carmine near the end of the film, his glasses start out being tinted, but when he takes them off, as Carmine hits and kicks him, the tint has gone; even outside they're tint-free.
Continuity mistake: At the beginning of the film when the girl is shot, in the close up there is no debris on her face, but the next wider shot shows her face covered with dirt and other bits of debris.
Continuity mistake: At the beginning, the little girl's body changes position. When the trucker finds her, her lips are slightly apart, but when the police examine her body, her lips are closed.
