Fallout 4

Bug: A game-breaking glitch can commonly occur during the prologue, upon creating a second character. *Spoiler* After the cryogenic chamber cutscene, after being released from the chamber, you will be stuck, unable to move or use any control except for Pause. The only known fix is to turn the entire game off and then go through the cutscene again.

Mafia II

Bug: There are several occasions throughout the main story where you may find a weapon with an outrageous amount of ammo ready to fire. Such as an M1 Garand with 1020 bullets in its magazine, and unable to reload as it has equal to or more than the amount of bullets at its max, allowing for nearly endless fire.

Bug: In the Batcave in the beginning, if you stun Robin with the sonar suit's sonar projector, and someone is playing as Robin, they will still be alive after being pushed off the side while stunned, and will be able to walk on the water outside of the playable area.

The Robbery - S4-E4

Plot hole: Towards the end, Jake (Shirley's boyfriend), hangs the girls on a coat rack by the hoods of their coats. They try a few comical ways of trying to escape, when they could have simply unbuttoned their coats and slip away.

Goodbye Island - S1-E9

Revealing mistake: While they are at the front or side of the damaged boat, you can see that the dark windows above them are obviously painted on, rather poorly in some spots, and not real windows.

Apocalypse Now (1979)

Trivia: Marlon Brando was giving director Francis Ford Coppola so much trouble towards the end of filming, that he actually left the filming of the final scene, where Sheen and Brando improvised the final moments without direction.
