Factual error: Ash receives a pistol as a weapon, which is a revolver. 5 or 6 cylinders by the looks of it. Yet Ash can spam shooting it constantly without reloading, like a semi-automatic with infinite ammo. It is not a supernatural weapon; it just somehow holds 50 rounds of ammo to constantly shoot without reloading in a revolver.
Revealing mistake: When closing the portals that spawn enemies using silver, Ash will stand there during the in-game cut scene as the portal shuts and do nothing. If an enemy spawns in the portal just before or as it's closing, it will start attacking Ash during the cut scene, and Ash just stands there taking hits until you're able to control him again.
Continuity mistake: Ash can sit there and bash on the NPC cops over and over with a shovel, and even slash them with the chainsaw, and the cop will fall down and yell stop. Then get back up completely uninjured and still have a conversation with Ash as if nothing happened.
Character mistake: When Ash talks to someone, a small scene plays out, and he will suddenly be standing there with nothing in his hand. But the moment the scene ends, he suddenly will still be holding whatever weapon he had before talking, like the shovel.
Continuity mistake: In the opening cinematic, Ash is dressed up and just fine, with no damage to his clothing and a complete blue shirt on. During that scene, he shoots one enemy with his shotgun and then goes outside before gameplay starts. Now suddenly Ash's shirt is all ripped to shreds, with parts of his chest showing and a sleeve fully missing. However, he hasn't done anything yet that would cause his clothing to be damaged. He shoots one guy and walks outside.
