Trivia: Director Christophe Gans has stated that his original director's cut was over two hours, but the producers made him cut it down to approximately 100 minutes. The director's cut contained more scenes and subplots, including another scene featuring Eddie and a bit more Pyramid Head, and is said to have a slower, more deliberate pace. Gans has stated he would like to release a director's cut one day, but is unsure if he can.

Trivia: The first cut of the film used archival music from the video game by Akira Yamaoka. Director Christophe Gans showed Yamaoka a rough cut of the film, and Yamaoka enjoyed it so much that he offered to score the film, writing some original music and creating a new version of the score from the games to fit the film's narrative.

Trivia: Unlike the previous two films, this movie was produced independently on a reduced budget as something of a passion project for director Christophe Gans. It was eventually picked up for distribution by studio Cineverse, who released indie horror films like "Terrifier", which is why it has a much smaller, quieter release than the previous films.

Trivia: Like the original 2006 film, director Christophe Gans mostly hired dancers to play the monsters in order to better portray their exaggerated movements. He also sought to portray the creatures in-camera (sans CGI) as much as possible.

Trivia: This film is loosely inspired by the 2001 video game "Silent Hill 2." In this film, the character Laura is played by Evie Templeton, who actually played the exact same character in the 2024 PS5 remake of "Silent Hill 2."
