Continuity mistake: When Mrs. Chavez is executed, the whole crime scene is wrong. Her body is about 3 feet from where she was shot (the revolver used was not sufficiently powerful to move her that far); the blood spatter is on a wall, not the floor; and is not spatter, but is painted on. The entry point is by her shoulder blade - there would not be that much blood, and the body she was cradling is nowhere to be seen.

Continuity mistake: A helicopter drops two harnesses down to Chuck Norris (Major McCoy) who is awaiting with an unconscious bad guy, and tethers the villain to the ropes dropped by the hovering copter. The black harness ropes are shown dangling from the chopper throughout. When McCoy radios to "pull us up" with the villain in view, the once visible ropes are nowhere to be seen.
