Visible crew/equipment: In the crushing room at the end of the film, Strahm is trying to open the box where Hoffman is. As it's descending into the ground, on the side of the box you can see the feet of a crew member with white sneakers moving around. It is not Strahm's feet: he's wearing black boots. No slow-mo is required.
Visible crew/equipment: During Batman's interrogation of the Joker, when he picks him up and slams him against the wall, for a very brief moment you can see the camera and the cameraman in the reflection of the mirror on the right. (01:29:10)
Visible crew/equipment: When Hancock returns Embrey and his car back to his house just after saving him and then picks up and moves the car onto the drive, you can see the blue crane used in the reflection of the car windscreen.
Visible crew/equipment: In the very beginning of the movie, Ben is seen riding his bike over the bridge in an aerial shot. Near the top of the screen, if you look carefully at the end of the bridge you can see a police barricade at the end of the bridge. It is there because the bridge is closed for filming.
Visible crew/equipment: During the car chase the driver's seat on Kent Taylor's car is located on the left side. When he finally gets shot by Barnes and is about to crash, in one of the shots you see a person (in all probability a stuntman) sitting on the right side. (01:17:10)

When the car drives in the alley and we see it from the top of the building (the camera angle), the camera filming them is visible in the reflection on the car's roof (it's mounted on a rod hanging high above the alley). Visible crew/equipment: In the opening murder scene watch when the man is stood outside the connecting train car doors, there is a shot from behind, moving in on him, and if you look out of the left window you can see a set of yellow ladders. This is set equipment and the train obviously can't be moving. (00:02:20)
