Revealing mistake: When McDonald is in the tunnels and is called to pick up the phone, he picks up a red phone. It is a standard Bell TouchTone desk phone. To make the phone look more futuristic, there was no cord and it was disguised as a cordless phone. The port that the cord would plug into is covered by a piece of colored tape. This also can be seen in the operations center of Ape Control when the worker picks up the phone.
Continuity mistake: The protagonist is weilding his karate skills against the crooked cops in the final fight scene. One of his foes is wearing a white fishing cap. Superfly belts him and knocks of the cap, there is a cut and then the cap is on again. The cap is on and then off again twice more in the same fight scene.
Visible crew/equipment: When Spencer and Hill fly the second air plane, the studio headlights are mirrored in Hill's sunglasses. (00:20:35)
Factual error: When Craig is being tortured in "New York," a spy operative threatens to plug in the electric shock machine in a European socket/outlet, rather than an American-type socket.
Plot hole: Shaft's friend has been killed at the beginning of the movie. Bad guys show up at his sister's door for a fake flower delivery, and Shaft kills them. He asks the dying baddie "Who put a contract out on me?", but how does he know? We as audience know there is indeed a contract on Shaft since we've seen the conversation with the mobster, but as far as Shaft knows, all evidence points at Arna being the target, and he even explained the money-related motive a mere minute before.
Revealing mistake: When O-Yuki is having a flashback to the duel she had with her trainer, at one point he "magically" sets his sword on fire. However, if you watch the scene carefully you can see a cord that runs from the sword into his robes, probably some kind of fuel line. You can see it twice: once in a wide shot showing the trainer and O-Yuki when he laughs and slowly raises the sword and a second time when he blocks O-Yuki's attack and punches her in the stomach. Revealing mistake: When Itto puts the cover on the baby carriage it's obvious that the sudden downpour of rain is just a sprinkler placed in front of the camera. Pay attention to the cover, Itto's clothing and the puddle behind the carriage, they remain completely dry/untouched. (00:11:00)
Revealing mistake: At one point when the soldiers are walking through the jungle, one of them steps into a trap and is pulled up into the trees to be killed by Indians. However, if you watch this scene carefully, when the soldier (who steps into the trap) walks into frame you can see a wire already attached to his foot leading up into the trees. He doesn't step into a trap. (00:22:26)
Continuity mistake: At the beginning of the movie, Bruce Lee enters a restaurant in the Rome airport and, because he can't read the menu nor speak the language, he accidentally orders five bowls of Campbell's Soup. When the waitress first delivers the five bowls, the soups are very different colors than they are in subsequent close-ups. Disappointed but hungry, Bruce plows into the soup. Ten sequential close-up shots show him dipping his spoon into bright red, tomato-based soups; but, by the time the spoon reaches his mouth, the spoon is dripping creamy white soup, every time.
Other mistake: It makes no sense that the pimp would die from having the tip of his tongue bitten off. (00:23:35)
