Corrected entry: In the scene in the kitchen at Dave York's home, just before McCall arrives to confront Dave York, there is one child at the table with York, and the other sitting in the background at the bar. As the scenes cut, the child at the bar is there, then goes, then reappears as the scene plays out.

Correction: The girl didn't just appear and reappear she walked to the refrigerator and then came back and sat down.

Correction: Susan is a former DIA operative. The implication is that she used her skills to get into the apartment undetected.

Also of note is that McCall is aware that someone has broken in and reacts to it as if it is a threat, until he discovers it is Susan.

Visible crew/equipment: When Denzel is fighting the man in the back seat of his car, they pass a silver Prius, which he hits. The camera tracks alongside. You can see the camera operator. It's clear enough you can make out that he is wearing a motorcycle helmet and has gloves on. (01:09:30)

Answer: It is never mentioned, but it stands to reason that eventually they did find out. York's crimes most certainly would have been made public, he is solely responsible for several murders including the murder of a police officer at the beach. There would be very little reason for the government to cover up his crimes since he was acting as a rogue agent.
