Continuity mistake: When Sam and Dean are tied up in the sewer, Dean gets out of the ropes holding him and the camera changes to Sam, but in the background you can see Dean with the ropes still around his arms & upper body.
Plot hole: When Sam and Dean realise they need to destroy the hook to stop the Hook Man, they find out that it has been reforged into something else. They state it needs to be set on fire to stop him, but if it was melted down and reforged then it has already been set on fire and the Hook Man shouldn't be latched on to it in the first place.
Continuity mistake: When Sam and Dean break into Lori's dorm, Sam notices (and the camera pans towards) an open window. The camera then cuts to Dean. When the camera cuts back to Sam the window is closed. We watch Sam open the window and climb through it.
Continuity mistake: In the very first scene with Lori and Rich kissing in Rich's car, Lori's seatbelt is shown to be over her arm close to her shoulder. The camera cuts to Rich and back and the seatbelt is now behind her arm and lower without her having moved at all.
Continuity mistake: When Sam gets the skeleton heads out of the back of the car, he brings the box in his arms long ways, but when the shot goes back to him it is turned the other way. (00:22:50)
Continuity mistake: Sam claims it is 12am and the the bug storm is on its way, but five minutes later the sun is shining. (00:37:00)
Continuity mistake: The baby gets into the fridge, which then closes - we see a closeup shot of the babyproofing clip shutting itself. Then when the mother sees the spilled milk and realises what's happened, there are a few shots of the fridge - in shots from the front the clip's open, and in shots from the side it's closed. When she goes to open the fridge it's open again - she even fiddles with it to give the appearance of undoing it before opening the fridge door.
Visible crew/equipment: When Sam tells the little girl to take her brother outside. The little girl runs out with her brother and Dean asks the little girl where Sam is, Dean runs to the Impala and grabs a shotgun. When Dean is running to the house, he runs on the lawn and you can see the shadow of the camera man running with Dean at the bottom of the screen.
Continuity mistake: When Dean reads about the Roosevelt Asylum in his dad's journal, there is a picture of a Wendigo on the opposite page of the newspaper clipping but in a different scene, Dean checks the article again for information on the South Wing in the Asylum but this time on the opposite page of the newspaper clipping, there is no picture.
Continuity mistake: Season 1 episode 2, Wendigo conflicts with this episode. Originally the wendigo information is printed on the left-hand side of the journal. On the right side is loads of symbols and text. Here the "same" page featuring the wendigo can be seen when Dean and Sam are searching for the asylum information but it's different.
Visible crew/equipment: When Dean thanks the professor before leaving the room, you can see the second camera pan down on the left side of the screen before it cuts to that camera's view. (00:28:40)
Visible crew/equipment: At the start of the episode, the couple comes out of the car and when it pans to show the orchard, a shadow can be seen walking in the lights.
Factual error: At the beginning of the episode it is stated that we are in the month of April - yet in the orchard there are ladders, fallen apples and baskets filled with fresh fruit everywhere, as if it were harvest time. The god is declared to grant fertility and not climate change, so the harvest decoration seems definitely out of season.
Revealing mistake: During the love scene between Dean and Cassie, you can see the tape used on Cassie to keep the show PG-13. (00:23:15)
Continuity mistake: Near The End of the episode when Dean is being chased by the possessed truck, Sam tells him to take a left. After he's made the left, Sam tells him to go exactly 7/10ths of a mile and stop. Dean looks at his gage and it reads 70098.3. Dean looks up, then back at it: it changes to 70098.5, but the next time he looks down it says 70100.6 and when he stops it says 70101.0, meaning he's gone 2 miles too far. (Probably meaning it took two shots to get the scene right but they messed up on the gage.) (00:35:00)
Revealing mistake: When Max opens the bedroom door to confront his step-mother and Dean, using his telekinesis powers, you can see the equipment used to open the door, a rope pulling the door on the floor. (00:34:19)

Factual error: Officer Kathleen looks for Sam's data in her database. The record ID is DF-23094. She then follows the link to Dean's record. Its record ID is also DF-23094. A database cannot have two records with the same ID.

Continuity mistake: When the Deputy handcuffs Dean to the car door handle, he uses a small radio/cell antenna from the car to pick his handcuff lock (attached to his wrist), just as the bad guys walk toward the car. When the bad guys get to the car Dean is gone and so are the handcuffs (including the cuff that was attached to the door handle). Dean did not have time to pick both handcuff locks.
Factual error: At the beginning of this episode, it says "one week later", then it shows Dean and Sam go to the victim's apartment. The blood is still bright red. After a week it would be darker.
Hell House - S1-E17
Visible crew/equipment: When Dean, Sam, Ed and Harry have just shot Mordechai, the cameraman's arm is visible on the right-hand side of the screen. (00:33:30)
Answer: Angels themselves may not have genitalia, but their vessels do. Nephilim occur when an angel in a vessel reproduces.
