Across whole show
Revealing: In season 3 episode "Fresh Blood": Dean cuts himself near the beginning of the episode, however the blood begins to drip (spurt really) before the blade cuts into his arm.
Continuity: In 'A Very Supernatural Christmas' (S3E08), when the Gods are beginning to sacrifice Sam and Dean, they slice Sam's right forearm. However, at the end of the episode, Sam has his left forearm bandaged, not his right.
Revealing: In season 1 episode 15, Officer Kathleen looks for Sam's data in her database. The record id is DF-23094. She then follows the link to Dean's record. Its record id is also DF-23094. A database cannot have two records with the same id.
Dead In The Water (season 1, episode 3)
Continuity: When the Carlton son reaches in to pull the plug out of the sink you can see the plug chain on the cutting board to the left of the sink before he even reaches in, then he pulls it out and puts the plug chain on the board.
Bloody Mary (season 1, episode 5)
Continuity: When Sam and Dean enter the Shoemaker home, a woman leaning against a wall holding a coffee cup starts to follow them down the hall. When the shot changes, the woman is back against the wall, talking to someone who wasn't there in the previous shot.
Skin (season 1, episode 6)
Continuity: During the scene where Sam is tied up and eventually fights the fake-Dean in Becka's house, there is a shot that shows both his feet and hands tied. During the fight, both his hands and feet are shown free, but he only cut free his hands, and began fighting immediately after that.
Continuity: When Sam and Dean are tied up in the sewer, Dean gets out of the ropes holding him and the camera changes to Sam, but in the background you can see Dean with the ropes still around his arms & upper body.
Bugs (season 1, episode 8)
Continuity: At the end of the episode, Dean and Sam pull up to the house to say goodbye. The moving truck has its back end facing the right, toward the front of the brothers' Impala. However, when they are driving away, heading straight without turning around, the moving truck is now facing in the opposite direction.
Home (season 1, episode 9)
Continuity: The baby gets into the fridge, which then closes - we see a closeup shot of the babyproofing clip shutting itself. Then when the mother sees the spilled milk and realises what's happened, there are a few shots of the fridge - in shots from the front the clip's open, and in shots from the side it's closed. When she goes to open the fridge it's open again - she even fiddles with it to give the appearance of undoing it before opening the fridge door.
Visible crew/equipment: In the scene where Sam tells the little to girl to take her brother outside. The little girl runs out with her brother and Dean asks the little girl where Sam is, Dean runs to the Impala and grabs a shotgun. When Dean is running to the house, he runs on the lawn and you can see the shadow of the camera man running with Dean at the bottom of the screen.
Asylum (season 1, episode 10)
Continuity: When Dean reads about the Roosevelt Asylum in his dad's journal, there is a picture of a Wendigo on the opposite page of the newspaper clipping but in a different scene, Dean checks the article again for information on the South Wing in the Asylum but this time on the opposite page of the newspaper clipping, there is no picture.
Scarecrow (season 1, episode 11)
Factual error: At the beginning of the episode it is stated that we are in the month of April - yet in the orchard there are ladders, fallen apples and baskets filled with fresh fruit everywhere, as if it were harvest time. The god is declared to grant fertility and not climate change, so the harvest decoration seems definitely out of season.
Route 666 (season 1, episode 13)
Continuity: Near the end of the episode when Dean is being chased by the possessed truck, Sam tells him to take a left. After he's made the left, Sam tells him to go exactly 7/10ths of a mile and stop. Dean looks at his gage and it reads 70098.3. Dean looks up, then back at it: it changes to 70098.5, but the next time he looks down it says 70100.6 and when he stops it says 70101.0, meaning he's gone 2 miles too far. (Probably meaning it took two shots to get the scene right but they messed up on the gage.)
Hell House (season 1, episode 17)
Visible crew/equipment: When Dean, Sam, Ed and Harry have just shot Mordechai, the cameraman's arm is visible on the right-hand side of the screen.
Croatoan (season 2, episode 9)
Visible crew/equipment: In the scene before Dean makes the promise to Sam, as the camera pans along the side of the Impala, in the rear of the car, you can see the dolly track that the camera runs along.
All Hell Breaks Loose (1) (season 2, episode 21)
Continuity: In the scene where Dean is with Bobby at the burned down Roadhouse and Dean gets Andy's vision about where Sam is, the first time Dean scrunches his face in pain he puts his left hand to his head. The shot changes to a back view and you can tell his left hand is still at his head. But when the shot switches again Dean is bringing his right hand down from his head.
All Hell Breaks Loose (2) (season 2, episode 22)
Revealing: Sam is breathing noticeably at the beginning of the show on the bed even though he is supposed to be dead.
Fresh Blood (season 3, episode 7)
Revealing: Sam takes the SIM card out of his phone so it can't be tracked, but when he takes the back off his phone, there's no battery, and the SIM card is just lying in the back not attached to anything! Same for Dean's - he takes the back off and the card just falls right out, not secured in any way as it would be in a real phone.
A Very Supernatural Christmas (season 3, episode 8)
Revealing: When the boys are tied up and the pagan gods are preparing for the ritual, the second time they cut into Dean's arm the knife is pressed along his arm and the blood is removed showing no cut at all.
