You Better Watch Out - S10-E10
Other mistake: McGee's text to Tony says "Dell's body found. Rock Creek Park." The suspect they are looking for and body found is named Justin Cannady. (00:22:23)
Revealing mistake: When Gibbs' flip phone started ringing, and he flipped it open to answer it, for the brief second, in the closeup shot of the flip phone, the screen on the phone says "No Service" despite receiving an incoming call.
Continuity mistake: When DiNozzo has got the victims watch on, it's on over his white glove. When the shot changes it's now over his bare skin and not the glove. (00:10:20)
Other mistake: When EJ gets spooked near the end and turns to face a wall with a doorway to "North Stairway C", she faces the camera with her torch facing the camera directly. However behind her is a perfect shadow on The Wall of herself. This shouldn't be possible as the only source of light, as shown, is from her torch. Cade and Levin are in a different room so couldn't be their torches. (00:39:20)
Answer: I hadn't seen the backdoor pilots yet but while it wasn't explicitly stated, at least onscreen, I heard somewhere Vivian got either fired from NCIS due to jeopardizing a case, got resigned from the agency, or transferred to the FBI.
