Plot hole: SPOILER Arlene Boras swaps Peter Magneri's MRI scans for those of a healthy person in order to hide the fact that he has a potentially lethal aortic aneurysm. Clearly visible on the MRI scans she is holding are the dates and times of the examination - and the name of the patient. Swapping the records is pointless.
Mr. Monk and the Voodoo Curse - S8-E7
Plot hole: The murderer planted the voodoo dolls after the accidents to make them look like victims and cover up her murder of her uncle, but none of that was necessary! She uses an untraceable poison on her elderly uncle to make it look like a heart attack. So if he'd died...they just would have figured it was a heart attack! No suspicion would have fallen on her anyway, the entire voodoo thing was just needlessly complicated and only exists for the episode's plot.
Mr. Monk and the Employee of the Month - S3-E7
Plot hole: Monk wants to take a look at the case that got Christie to leave the force. But given that they are ordinary cops they would not be able to just go and pull internal affairs files.
Suggested correction: I'm not sure what you mean by "ordinary" cops. Captain Stottlemeyer is the one pulling the file. Plus, we don't see what they had to do to request access the files. There's no indication they just went in and randomly pulled the file with no authorization.
Mr. Monk and the Astronaut - S4-E14
Factual error: During their examination of the crime scene, neither Monk, Stottlemyer, Sherona, Randy or anyone else in the room wear gloves or any kind of other protective clothing. They handle artefacts that could be critical to an investigation of the woman's death, including a cheque made out to her, and they hopelessly contaminate them by doing so.
Mr. Monk is the Best Man - S8-E13
Character mistake: At the end, after Stephanie fired the gun, Captain Stottlemeyer shows her the gun's magazine but calls it a "clip." A police captain like Leland would know the difference between a clip and a magazine and would use the right word.
Mr. Monk and the Rapper - S6-E2
Factual error: Extra Large was killed Sunday morning of Daylight Saving's Day. But the paper they show Monk is dated Friday, May 4, 2007. But Daylight Saving was March 11, 2007.
Mr. Monk and the Candidate (1) - S1-E1
Factual error: When the rifle is identified as a "Weatherby Fibermark", Monk and Randy say it's a British rifle. But Weatherby is an American company.
Mr. Monk and the Sleeping Suspect - S2-E7
Continuity mistake: Monk throws the bomb into the toilet and shuts the door, pulling it towards him. When we see him hit by the blast there's no door in the way, and in the reverse shot looking into the bathroom the door has swung open into the room again, despite the blast radiating outwards. Plus it should have destroyed the door anyway.
Mr. Monk and the Airplane - S1-E13
Factual error: When the flight attendant announces that "they have been asked to return to the gate" the French pilot and his mistress are already celebrating with a glass of wine, but that is impossible since drinks are only served after the take off phase when the plane has reached its traveling altitude. Mr. Monk and the End - Part II - S8-E16
Continuity mistake: Trudy mentions that she had a baby fathered by her professor on Jan 2nd, 1983, before she and Adrian met and 15 years before her death on Dec. 14th, 1997. However, according to the storyline (and the episode "Mr. Monk and the Reunion"), Adrian and Trudy had first met in college, both being in the graduating class of 1981.
Mr. Monk Goes to Jail - S2-E16
Continuity mistake: One of the times the prison guard is attempting to pat down Monk, his hands are approaching Monk's abdomen. After a cut to the next shot, his hands are suddenly around Monk's lower legs. (00:05:55)
Mr. Monk Goes to the Hospital - S5-E16
Character mistake: The doctor remarks that his patient has rheumatoid arthritis early in the episode, and later he says osteoarthritis. These are two different conditions. (00:19:15 - 00:28:40)
Mr. Monk Goes to the Hospital - S5-E16
Factual error: The doctor says Monk is losing "cranial fluid" when speaking to the interns. He means "cerebrospinal fluid." There is no such thing as "cranial fluid", especially in medicine. (00:28:50)
Mr. Monk and the Other Detective - S4-E1
Continuity mistake: The body in the car park (covered with a yellow sheet) moves around from shot to shot. Sometimes it's parallel to the parking bay lines, other times at an angle over them, or at an angle next to them.
Mr. Monk and the Naked Man - S6-E3
Character mistake: SPOILER. Arlene Boras murders Vicki Deline in order to stop her telling Peter Magneri that he has an aortic aneurysm. Boras wants Magneri dead to clean up by short selling the stock in his company. Boras is a medical professional, and she would know that an aneurysm can burst if the patient is under stress or is shocked in some way. Allowing Deline to tell Magneri that he is in danger of dropping dead on the spot would kill him, which is exactly what she wants.
Mr. Monk and the Sleeping Suspect - S2-E7
Plot hole: Technically, Brian Babbage's plan would never have worked due to the fact that all outbound mail goes to a main post office from the local post office and then placed through scanners to determine the contents, which would have inevitably exposed the bomb inside.
Continuity mistake: In "Mr. Monk and the Birds and the Bees" Natalie mentions she doesn't have a sister when Julie's "boyfriend" says the aunt hired him to break her and her real boyfriend up. Here she has a sister and a niece.
Suggested correction: Natalie doesn't have a sister in this episode. The woman seen is credited as "Aunt Teresa." They're most likely cousins. Children of cousins are often referred to as nieces/nephews. This would make Natalie "Aunt Natalie" to Anne Marie (and Teresa would be "Aunt Teresa" to Julie).
Mr. Monk and the Naked Man - S6-E3
Character mistake: SPOILER ALERT Monk accuses Arlene Boras of swapping Peter Magneri's X-rays in order to hide the fact that he has a potentially lethal aortic aneurysm. We see Magneri being examined and we see Boras handling his records, and they are not X-rays. Magneri was examined in an MRI scanner and Boras swaps the MRI scans - not X-rays.
Mr. Monk and the Big Game - S5-E3
Factual error: The hair dryer would not have hurt anyone as it was plugged into a FCI outlet. These outlets will immediately sense the interruption and cut the power to ensure electrical safety. The coach would have been alive. (00:02:23)
