Character mistake: At some point Monk asks his father what the device in the car is. His dad tells him it's a GPS. It's difficult to believe that our detective doesn't recognize the machine, even if it's probably another model: the whole plot of the episode 2x03, Mr. Monk Goes to the Ballgame, revolves around a tampered-with GPS.
Happy Birthday, Mr. Monk - S8-E9
Character mistake: Given how Monk is it seems unlikely he'd forget about being in a dumpster filled with garbage and be able to enjoy a birthday party, even if it's his own.
Mr. Monk and the Lady Next Door - S7-E12
Audio problem: Two words near the beginning of Monk and Marge's dinner conversation are audibly spliced. When Marge says "what does a", the word "a" is cut off at the beginning; and when Monk says "detective does" in his response, the end of the word "does" is cut off during the s. (00:15:15)
Mr. Monk Is on the Run (2) - S6-E16
Plot hole: When Stottlemeyer is confronted by the Sheriff for not going to the morgue there are 2 plot holes in that one scene: first it is impossible for that article to be in the morning paper since the coast guard "just fished him out" last night. And secondly an article, especially a long article like the one about the body in the bay would contain a physical description of said body. That is the reason it does not make sense to even ask the captain why he did not go to check the body to see if it was Monk when the body was tattooed and had red hair. (00:13:30)
Suggested correction: The article being there that fast is a bit odd, the sheriff mentions a police alert had been put out and was why he went to the morgue. That was what he was confronting the captain about. Even without the article the point of why he didn't check is still a valid question.
Do you really believe that the police alert did not include a description of said corpse being found for the missing person's unit to combine against their lists?
Mr. Monk and the Three Julies - S6-E13
Plot hole: There is no other reason for Randy to hand Natalie the walkie-talkie but to fill that plot hole. He is in fact in breach of police rules when he hands his department issued radio to a civilian to answer it while he is on duty. The same goes for Stottlemeyer and the car keys he hands to Monk. Even more so since he knows Monk could not even drive that car to the side in the far fetched scenario that a police car had to be moved while the cop is serving a warrant inside the building the car is parked in front of. Factual error: Though anti-freeze is lethal it does not kill quietly and fast but rather agonizingly, painfully and slowly, inducing vomiting and fever. (00:30:30)

Other mistake: The camera, although it is supposed to be stationary, is visibly shaking during the shot of Monk and the woman discussing the painting. (00:23:45)
Audio problem: When Natalie is saying "it" in the phrase "I still don't hear it", her voice sounds different than in the rest of the sentence, as it was audibly spliced in two. They used two different takes and combined them. (00:03:45)

Continuity mistake: There is a shot where Randy is trying to cross the police tape and an officer stops him, and he can be seen briefly arguing. However, when it cuts to the next shot, we see that argument happening again in the background, repeated. (00:07:20)

Continuity mistake: Monk pushes aside a glass of milk on the table in his apartment. It is mentioned multiple times during the series that Monk is afraid of milk. He would never have any in his home, and certainly wouldn't have a neutral reaction to it. Also, the milk appears to have spilled a little as it was pushed, and yet in the next shot there is no milk on the table. (00:33:00)

Audio problem: In the last line of the episode, the end of the word "spider" that Natalie is saying is cut off. It sounds like "spi." (00:42:05)
Suggested correction: I just watched the scene and I hear the whole word "spider."

Mr. Monk Goes Back to School - S2-E1
Continuity mistake: Mr Monk completes his name on the chalk board at the beginning of class, but when class is dismissed the 'k' in Monk is incomplete.
Suggested correction: After he wrote his name, he says "what's up with that k?" and erases it. He started to re-write the k when he got hit by the eraser, leaving it unfinished.
Mr. Monk Goes to the Hospital - S5-E16
Audio problem: When Monk is saying "motive" in the phrase "this one had a motive", the word is spliced in two, as they used two different takes and combined them. Very robotic. (00:28:55)
