Enter the Demon - S4-E4

Plot hole: When Yen lo enters the manor in the attic. Where did the water on the floor he comes through come from? Windows were closed, no visible leak, and the sister took precautions to make sure there was no water in the house. Also after he attacks and gets the dragon blade, he exits the house through the sink filled with water. Piper had already said to make sure there was no surfaces of water in the house and had Phoebe and Paige shut the water main off.

Suggested correction: There is a previous scene that shows a visible leak in the windows and an obvious puddle of water on the floor of the attic. This is the same puddle they later enter limbo through. As for the sink, Phoebe (in Paige's body) explains that she had been soaking potion pots and forgot to drain the sink. Turning off the valve wouldn't dissipate water that is already in the sink.

Enter the Demon - S4-E4

Other mistake: Yenlo explains that he escaped limbo through the water in the clouds. And the yen master says there is no other water around. So why do the girls come in through a cave? If they have to leave through the water in the clouds they should enter through the clouds.

Size Matters - S4-E5

Continuity mistake: Shortly after Phoebe frees the blonde girl from the clay, she tells her to run "before Gammill sees [them]." But Phoebe hadn't learned the demon's name at any point. Leo and Piper do find out his name, but not until later. (00:20:00)

Suggested correction: Gamill and Finn are talking when Phoebe is at the house. It may be possible that Gamill's name was mentioned by Finn in conversation.

Size Matters - S4-E5

Continuity mistake: Piper suggests that Paige orb in place, because the clay is still wet enough that it will collapse if she's not there to support it. But when Paige actually does orb, the clay cracks and collapses into powder as if it had been bone dry.

Suggested correction: Piper says in the episode that she allowed him creative control because the club wasn't doing well, and he was well-known to be good at what he does. It's made clear throughout the episode, by way of him telling her multiple times that if she doesn't like it that he'll get rid of it, that she still has full veto power on any changes.

A Knight to Remember - S4-E6

Plot hole: Piper is initially skeptical at Phoebe's theory that the Evil Enchantress could be Paige in a past life, because Paige lacks the Enchantress' ability to summon the elements. However, Piper knows that in Phoebe's past life she had a pyrokinetic power, which she does not have in the present.

Suggested correction: This is not a correction, more of an observation that goes with this plot hole. In season 2 episode 14 'Pardon My Past' Leo explains to the sisters that Phoebe had her active powers taken away, because her past self had used them for evil, but yet the Enchantress does the same with her powers, and Paige still has active powers. It took two years for Phoebe fighting as a good witch to actually gain an active power again, and then it was only levitating. Becoming an empath took seven years. So that doesn't really make sense that Phoebe would be punished for her past self actions and Paige was not.

Possibly Paige had past lives in between the one we saw and the present one. So she could have already atoned for the evil past life in another past life.

That could very well be true since Paige's past life as the Enchantress was much further back in the past than Phoebe's.

Also Paige's active power comes from her Whitelighter side not her witch side, she doesn't have any active witch powers.

True, but Paige's past life, the enchantress, was medieval, maybe other versions of her past life post-medieval times were good witches? Phoebe's past life was more recent like in the 1930s.

Paige's active power comes from her whitelighter heritage, so like Chris and Wyatt, she doesn't have an active witch power, so this active power has nothing to do with her Wiccan heritage and past life as the enchantress.

A Knight to Remember - S4-E6

Plot hole: According to Paige's "fairy tale" the witch ends up with the prince. If this is really a past life, the sorceress ended up with the prince in the Middle Ages. So why do they have to fix it? Everything was just going the way it did according to her fairy tale, and good magic was obviously still present in the world.

A Knight to Remember - S4-E6

Plot hole: When Phoebe tried to kick the Shocker Demon, she became electrocuted until Piper interposed with a large wooden stand. But when the prince attacked the Shocker Demon with his metallic sword, the demon was cowed and the prince was unharmed.

Black as Cole - S4-E8

Continuity mistake: Paige looks in the Book of Shadows and suggests that the demon they are looking for could be Belthazor, not knowing that Belthazor is Cole's demonic form. But we saw Phoebe making extensive changes to Belthazor's entry in the Book of Shadows in a previous episode, and the changes are not in evidence here. (00:01:40)

Continuity mistake: After Phoebe finds the Scavenger Demon page in the Book of Shadows the camera dollies out, and when Piper asks what happened with Cole, the crew's taped black mat is visible on the bare floor with no fringed area rug, then when Cole shimmers into the attic, in the next shot of Piper and Phoebe we see the fringed area rug at the bottom of the screen. Plot hole: After Cole explains he might not be able to regain control after embracing his demonic half, he tells Phoebe she should have the Belthazor vanquish potion ready just in case. But in "Look Who's Barking", Piper tried using that potion on Cole and it didn't work because he had himself magically protected from it. (00:27:15)

Continuity mistake: When Phoebe tries talking to Emma about Belthazor, Emma's hands are folded in front of her, holding her sweater closed. In the following shot when Emma turns around, her arms are now down by her sides, with her sweater open. (00:31:10)

Continuity mistake: After Phoebe jump kicks a warlock and falls to the ground, there is a blonde woman standing next to them. In the next shot of the warlock, when Piper blows him up, the woman is gone. But all of the mortals had been frozen, so she couldn't have moved. (00:37:40)
