Mistake Continuity: When John is driving to work and he hears Anna on the radio, he swerves to avoid a car. From inside the car you can see that there are no cars in the rear window, yet when the shot changes to an outside shot, there are 3 cars.

Mistake Visible crew/equipment: About half way through there is a shot that shows Jonathon sitting in his living room, On the right hand side of the screen we can see the windows and it's dark outside. Even so, you can see a camera man holding a camera moving along the window filming Jon from behind.

Mistake Continuity: In the scene where John first meets Raymond, Raymond sticks his business card in the black slot. It is big enough that it hangs over the slot. When the camera pans away from the card to go back to their conversation, it disappears. At the end of the scene it appears again, because they end on a close-up of the card.

Mistake Continuity: In the scene where Michael Keaton gives the blonde woman medication to help her sleep, and she reclines on the bed, as she reclines her legs are turned to her left, but in the next shot, she is speaking to him while lying on her right side.

Mistake Continuity: When Keaton is watching Anna take his son to school he is standing in the driveway. During the first shot his cell phone is on a clip on the side of his pants. When the shot changes to Anna in the car, then back to Keaton, his cell phone has moved to the front from the side.

Mistake Continuity: Also when John is driving to work to review the tapes, the camera is following an SUV through the intersection which implies they are following his vehicle. That vehicle is a Honda CRV, but when he swerves to miss the oncoming car, the vehicle switches back to a Ford Explorer.

Mistake Continuity: After Jonathan saves the baby from the car he goes to the funeral. In the funeral he starts talking to the baby's father and while they are talking you see a bald man with glasses pass by and glance at the baby and then in another scene you see the same bald guy pass again.

Mistake Continuity: During the car crash scene, the sparking transformer starts to buckle and come away from the telephone pole, but in the all overhead shots of the pole, the transformer is back in its original position.

Mistake Audio problem: When John Rivers is installing his new audio equipment, he says to his son "So you can watch your favourite show and I can watch my favourite show". John's mouth movements don't match what he is saying.

Mistake Visible crew/equipment: In the bookstore, when Sarah is passing boxes down to Jonathan, the boom mic drops into frame for several seconds.

Mistake Factual error: When looking at Anna's cell phone, you can see the logo for Telus, a Canadian communication company that only offers cell phone service in Canada.

Mistake Continuity: When we see Anna as a 'ghost,' she is wearing a flowing, lightly colored garment, NOT what she was wearing when she died. But when we see John's ghost, he's wearing the clothes he died in, as you can assume most of the others were as they seemed to be wearing casual garments. The killer (as you could see with his last attempted victim) did not change his victims' clothing before killing them. Why then, was Anna wearing what seemed to be a night gown?
