Please note: Almost all the timecodes are from the Extended edition. Because it's split across two discs this makes timecodes rather tricky, so unless stated, the timecodes are from the beginning of the relevant disc. I know this causes confusion when ordering by time, but it's easier when actually watching them.
Continuity: When the Balrog's whip wraps around Gandalf's ankle and pulls him off the bridge in Moria, you can see that the surface of the bridge where he was standing is perfectly flat and smooth. We cut to Frodo screaming, then back to Gandalf, who is now hanging onto an easy-to-grip groove in the rock.
Continuity: When Frodo rescues Sam he grabs hold of his wrist and pulls him into the boat. In the shot in which Frodo grabs hold of the side of the boat with his left hand while pulling Sam into the boat with his right hand, we see the large brown mat on the floor of the boat. In this shot the mat is under Frodo's legs and Sam's sword is not at all under his cloak. In the following shot behind Frodo as Sam is trying to sit up the mat is not under Frodo's legs but only under Sam, and Sam's sword is now entirely under his cloak.
Continuity: When Boromir blows the Horn of Gondor in the close-up (the second time he is seen doing it), slung around his neck are both sides of the horn's attached rope in front of his torso, as he drops the horn to grasp the sword with both hands to fight the Uruk beside him. In the following shots it varies, the horn is either slung over the same way or it is slung over his shoulder, with one side of the rope at his back!
Continuity: When Haldir says, "Welcome Legolas, son of Thranduil," the outfit that Legolas wears is not the same in these shots than in the previous and following shots. Gimli's hair/beard are not as full and his prosthetics are a bit different too. Apparently this scene was one of the first to be filmed, but they realized later that this outfit wasn't working for the role of Legolas, so they changed it. Commentary on extended DVD.
Continuity: After spotting the Crebain from Dunland, the Fellowship try to hide. Just after Frodo and Aragorn hide under some boulders, there is an aerial shot of the area. Frodo, holding the green cloak, can be seen ducking behind the tall boulder on the left, even though he's shown already hiding in the previous shot.
Revealing: Gandalf and Bilbo sit on the bench outside of Bag End smoking their pipes with the party area in the distance. The tree, the water, Hobbit home (future home of Sam and Rosie) to the right and party area is extremely different in dimensions and aspect ratio, compared to the background that's seen when invisible Bilbo comes through the gate at Bag End. It's also very apparent, because of the hair, that it's Bilbo's scale double in the back shots on the bench.
Revealing: As Boromir blows the Horn again at Amon Hen, in the first wide shot, he's battling an Uruk. In the close-up, he slices the whole arm off that Uruk, just under his shoulder, which starts to spurt, and the dismembered arm isn't seen. In the next wide shot, we see the whole arm of that Uruk still there and he is just dropping his sword out of his hand, then falls to the ground.
Visible crew/equipment: After Sam dives between the Troll's legs and tries to crawl away, in the first shot the Troll lifts his leg and in the second shot he's about to step on Sam who is in the middle of the floor. Yet in the very next shot Sam is already at the wall raising his left arm and wincing at the giant foot coming down on him. Aragorn and Boromir pull the chain that is around the Troll's neck to get him away from Sam. Then there's another shot of Sam. In these two shots Sam's left wrist has an ace bandage wrapped around it - it's at his white shirt cuff. Notice the bar on his palm between his thumb and index finger. He did not have the bandage in earlier shots nor does he have it in later shots.
Continuity: At Balin's Tomb, Pippin backs up towards the well, we see the pail and there is a thick bunch of chain hanging out of it. In the next shot of Pippin as he backs up and turns, the pail now has one line of chain hanging out of it. Then Pippin touches the quiver of the arrow, and in the first shot of the head falling back into the well the pail yet again has a thick bunch of chain hanging out of it. Then in the next shot of Pippin turning around towards Gandalf the pail once again has the one line of chain hanging out of it.
Continuity: In Rivendell, when Bilbo gives Frodo Sting, as he's unwrapping the sword from the cloth, in the close-up he spreads the cloth out flat on the bench in front of the bed. In the next shots, as he shows and then gives Sting, the cloth is wrinkled up on the bench. (Time appropriate for second disc.)
Continuity: Pippin is falling down the hill after the incident with Farmer Maggot. In the shot from above, we see he is about to land with his chest right on top of the pile of dung. Yet, in the successive side shot of him, the dung is obviously not beneath his body, and we see in the next shot of his face, that the dung is now in front of his face, for him to remark, "Oh, that was close!"
Continuity: In Farmer Maggot's field, when Merry and Pippin drop in on Frodo, in the first shot, as they are bending down lifting the vegetables off the ground, Merry and Pip are standing side by side, Merry on Pippin's right. Opposite them are Frodo and Sam, with Sam on Frodo's right. In the next shot, when they're upright, Frodo and Sam are across from each other, as are Merry and Pippin, with Pippin on Sam's left. In the next shot, Pippin is now on Sam's right. The change in positions has nothing to do with Merry and Pippin lifting the vegetables off the ground.
Continuity: After hearing noises out the window, Gandalf tells Frodo to, "Get down," and reaches for his staff leaning on the mantle. In the close-up when he grabs the staff, on the mantle there are books on the left end with a box near it, and a piece of paper leaning up against the wall between the two. In the wide shot just before this shot, there is no paper between the books and box. Also, the paper lying between the books changes from previous shots and when he starts heading for the window, behind him on the mantle the paper to the right of the candlestick changes position in the following shots.
Continuity: During Bilbo's party, in the first shot, as Merry boosts Pip onto the back of the wagon, on the table in front of the tent there are pears, oranges and apples on it. Once Pippin is in the wagon, seen behind Merry, there is one red apple that has appeared at the front of the table to the left of the other fruit that wasn't there before.
