Please note: Almost all the timecodes are from the Extended edition. Because it's split across two discs this makes timecodes rather tricky, so unless stated, the timecodes are from the beginning of the relevant disc. I know this causes confusion when ordering by time, but it's easier when actually watching them.
Revealing: The braids over Legolas' ears on his scalp, change drastically in position, shape, amount and how it is braided, as well as the amount of his side hair that is swept up into the back braid, in consecutive scenes or shots. It's impossible for him to change his hair between the following shots. In Balin's Tomb, after Pippin's carelessness, when Legolas turns around with Aragorn to look at the door. When he aims his arrow at the door, waiting for Orcs. When he shoots the last arrow into the Troll's mouth. After leaving the Tomb, when Gandalf says, "A Balrog of Morgoth." When he leaps across the broken stairs. Catching Aragorn after Aragorn drops from the broken stairs. While he stands outside Moria, after losing Gandalf.
Continuity: The Fellowship wait for the Orcs and Troll to break through the door at Balin's Tomb. In the wide close-up shot of the door we see the two large axes lying across the top of the door handles, and two spears leaning against the doors. Gimli says "There is one Dwarf yet in Moria who still draws breath" and we see the doors in the background with no spears leaning on them. In the close-up again the spears are there. In the next extreme close-up the large axes' handles are lying across the doors behind one another. There is yet another extreme close-up and now the large axes' handles are lying across the doors on top of each other. In the next wide shot the Orcs break through the doors and the left door starts to crookedly come down. Then in the next shot the doors are just now being broken through, only this time the left door comes straight down.
Continuity: After hearing noises out the window, Gandalf tells Frodo to, "Get down," and reaches for his staff leaning on the mantle. In the close-up when he grabs the staff, on the mantle there are books on the left end with a box near it, and a piece of paper leaning up against the wall between the two. In the wide shot just before this shot, there is no paper between the books and box. Also, the paper lying between the books changes from previous shots and when he starts heading for the window, behind him on the mantle the paper to the right of the candlestick changes position in the following shots.
Continuity: At Bag End while speaking with Gandalf, Bilbo packs his bag and he puts his pipe into it. The mouthpiece goes into the bag with the bowl part sticking out of the bag. Yet, when Gandalf asks, "Is that so hard?" the mouthpiece is now sticking out of the bag and the bowl part is in the bag.
Continuity: At Bag End, with his left hand Bilbo holds a plate with pieces of a sponge cake. The small part of the sponge cake is to the left of his thumb, and the large part is opposite his thumb. When there is knocking at the door, the smaller piece is to the right of his thumb. When he's at the window, the smaller piece is to the left again. As he runs into the kitchen, the large part of the sponge cake is now by his thumb. The plate never switched hands.
Continuity: When Gandalf reaches Bag End, in the wide shot while he's still in the wagon we don't see certain things that are seen in the next close-up shot when Gandalf stands at the front door. Seen in the close-up, the short small pink and purple flowers that are located on the ground to the right and left of the front door. Also, the large tall pink flowers growing against the wall to the left of the door seen behind Gandalf in this shot.
Continuity: After Gandalf arrives at Bag End, in the wide shots looking through the doorway of the kitchen, the white cloth on the mantle over the fireplace hangs twice the length as it does in the next close-up, when Bilbo says, "I can make you some eggs if you'd-" Then, while they're in the kitchen, the white cloth hangs longer again.
Continuity: At Bag End, the table opposite the fireplace with the two oval portraits changes in the variety and arrangement of all the objects on it, seen in shots that are supposed to take place right after each other. In the first shot, when Bilbo packs and then takes his bag off the table, we see what is on the table. The second shot, when Gandalf stands to confront Bilbo to leave the Ring, the table is seen to his right and we see what is on the table. The third shot, when Gandalf sits by the fireplace and waits for Frodo to come back from the party, the table is to his left and again we see what is on the table. The objects differ in every shot.
Continuity: When Frodo and Sam are in the field with the scarecrow, in the close up shot they're a lot closer together than in the wider shot. Nothing to do with perspective - check out the amount of green corn in front of Frodo in the two shots. The scarecrow also changes place and it's sometimes next to some corn and sometimes not.
Revealing: When Arwen is being chased on horseback through the woods by the Ringwraiths, at one point her face gets grazed by a tree branch, which leaves a long red scratch on her right cheek. In the following shot from up behind the horse she's riding, when she turns around one more time to see where the Wraiths are, her face is conspicuously unblemished, only to have the scratch re"surfaced" on subsequent close-ups. Also, during this long flight to the Ford sequence it is quite discernible in many cuts by cueing on her face (esp'ly in slo-mo) that the rider is a stunt double.
Continuity: In the scene in Isengard, when the Orcs are digging the Uruk-Hai out of the mud, it shows a Uruk-Hai grabbing the throat of an Orc. When he first grabs him, and it shows the Uruk-Hai, its got relatively clean shoulders. When it shows him again, there are piles of mud on his shoulders and chest. Than when it shows him standing up, there is no mud on his shoulders or chest, but he is covered in slime.
Continuity: When Frodo rescues Sam he grabs hold of his wrist and pulls him into the boat. In the shot in which Frodo grabs hold of the side of the boat with his left hand while pulling Sam into the boat with his right hand, we see the large brown mat on the floor of the boat. In this shot the mat is under Frodo's legs and Sam's sword is not at all under his cloak. In the following shot behind Frodo as Sam is trying to sit up the mat is not under Frodo's legs but only under Sam, and Sam's sword is now entirely under his cloak.
Continuity: After the battle at Amon Hen, Frodo rescues Sam from drowning and pulls him into the boat. During their dialog in the boat, you can see that the boats' stern is pointed toward the bank of the river. There is one brief view of them from the riverbank where the side of the boat is facing the riverbank.
Continuity: When Boromir blows the Horn of Gondor in the close-up (the second time he is seen doing it), slung around his neck are both sides of the horn's attached rope in front of his torso, as he drops the horn to grasp the sword with both hands to fight the Uruk beside him. In the following shots it varies, the horn is either slung over the same way or it is slung over his shoulder, with one side of the rope at his back!
Revealing: When Gandalf and Saruman fight at Orthanc, Gandalf causes Saruman to slam backwards into the wall and then Saruman makes Gandalf spin horizontally in mid-air. In the next shot, when Gandalf falls, the wrap around his left hand is visible, yet, he does not wear it in the previous and following shots.
Continuity: During Bilbo's party, in the first shot, as Merry boosts Pip onto the back of the wagon, on the table in front of the tent there are pears, oranges and apples on it. Once Pippin is in the wagon, seen behind Merry, there is one red apple that has appeared at the front of the table to the left of the other fruit that wasn't there before.
