Continuity: When Luc Deveraux and Veronica Roberts are on the prison bus and Andrew Scott is chasing them on the truck/base of operations Andrew starts throwing hand grenades. Andrew throws one that lands inside the bus. Veronica has time to fumble around for it, pick it up, scream about it and throw it out of the window before it explodes. The rest of the grenades that Andrew throws explode in a couple of seconds like they are supposed to.

Continuity: In the hotel scene when Van Damme is crashing through walls to escape the other Universal Soldiers who are shooting from outside, one shot from the inside of the room shows him hitting the wall with his left shoulder, but when the camera goes into the next room to show him coming through that wall, he is breaking through with his right shoulder.

Audio problem: When Jean Claude and the girl are in the bus while Dolph is throwing hand grenades, they say a few lines as their bus is getting rammed. There are 3 lines in a row where what they say is not what is heard. 2 lines by the woman and 1 line by Van Damme.

Continuity: In one scene JCVD is trapped in the back of an out of control prison bus. The driver has been killed but has, one must assume, died with his foot jammed on the pedal because the bus doesn't slow. In a shot facing back at the bus you can clearly see a pair of hands steering the bus.

Continuity: After Veronica's car crashes, she is seen on a CCTV screen showing her ID. Half a second later, when the angle changes, her hairstyle is different.

Continuity: After escaping from the motel, Luc and Veronica buckle up. Minutes later they pull the car over and in the wide angle you can see that neither he nor Veronica is buckled up. Then in the close-up they're buckled up again.

Plot hole: When Luc and Veronica escape from the motel, they pass by an old lady reading a newspaper. Given that 4 soldiers have been bombing the place for the last 10 minutes it is totally idiotic to think that the old lady would be calmly sitting in the middle of the chaos.

Continuity: Veronica and his mate arrive to the military base, pull over and chat. From her window the scenery is dark and still, but from his it's daylight and the car is moving.

Other: When Lundgren throws a grenade at a Vietnamese woman, although the camera is always focused on her, the grenade is never seen falling. There's just a sudden explosion next to a bunch of bushes and debris, most surely where the pyrotechnics were hidden.

Continuity: When Luc is in the tub covered with ice bags, a bag appears next to his armpit in the close-ups, but disappears in the wide angles.

Continuity: When the waitress asks Van Damme if he's got money to pay for the food, the position of her left arm and hands keeps changing between angles (bent, raised, lowered).

Continuity: When the huge truck is pushing the prison bus towards the cliff with Luc on top, Luc disappears in the wide angles, and magically appears in the close-ups.

Continuity: When Lundgren bombs the prison bus, the aerial shots and the close-ups have nothing in common: In the aerial the bus is in a very narrow space next to a huge cliff, with nothing around but sand and rocks. In the first close-up the bus is rolling in a wide field, no sign of the cliff. In the second close-up a wooden fence and a house-shaped sign suddenly appear, only to disappear in the third close-up.

Continuity: The prison bus has a huge blood splatter next to the driver's seat, which changes to a line of blood seconds later.
