Visible crew/equipment: In the episode "Head over Heels," twice you can see the cameras mounted on the walls that are used to film this series. The first one is during the cross-examination of Miss Morris and the second time is during the defence summing-up.
Visible crew/equipment: When Ned walks upstairs to check on the lady before the bad guy gets killed, behind him is a circular window. A reflecting light can be seen in the window along with what looks to be a boom microphone.
Visible crew/equipment: Toward the end of the episode you can see a guy with a camera reflected in the side of a silver car.
Rumpole and the Fascist Beast - S2-E4
Visible crew/equipment: While Rumpole's wife prepares the canned soup in the kitchen, several equipment shadows are moving on the wall at the top of the screen. (00:06:40)
Visible crew/equipment: In the flashback of the murderer slamming the car door on the victim's neck and breaking it, we get a clear shot of the cameraman and his camera in the car door (CSI's favorite blooper).
Visible crew/equipment: Towards the beginning, when the Taliban steps on the landmine, you see the DEA agent getting hit with dirt and blood. In the center of the shot, you can see a few drops of blood stick to the camera lens. Incredibly easy to notice, so I'm not sure how this one slipped through.
Visible crew/equipment: At the end of the episode Saul is sitting in his office and you can see the shadow from a crew member in the windows. The member even moves. (00:50:41)
Visible crew/equipment: Neelix says to Chakotay 'I don't understand, it's right here,' referring to the dilithium ore. As soon as the camera changes to show him walk into the large cavern, a boom microphone is quickly pulled away from covering his face. (00:07:45)
Into That Good Night: Part 2 - S9-E24
Visible crew/equipment: When Bev hears "movement" on the baby monitor everyone rushes toward Roseanne on the stairs, and just as Roseanne comments that they're all "scary enough individually," we can see the second camera appear at the left side of the screen. Then, when Bev walks through the bedroom's doorway, there's movement of the boom mic at the top left corner of the screen, and when DJ turns on the camcorder the end of the set is visible from the POV of the camcorder.
Visible crew/equipment: When the boys are picking teams for basketball, a boom mic can be seen in the top left corner of the screen. (00:06:20)

Visible crew/equipment: When Flores discovers her cell phone is missing from the hole in the wall she goes nuts, slams the stall door and kicks it in. After she is taken away there's a closeup of the stall and we can see the metal spring that was duct-taped to the bottom of the wall inside.
Visible crew/equipment: After Echo is extracted from the docks, the reflection of a camera rig can be seen on the side of Langton's van. It's within the last few frames of the shot so you may have to pause it to see.
Visible crew/equipment: When Kathryn tells John that staring at his phone won't make it ring, in the next shot facing John the boom mic can be seen in front of him, bobbing around at the top of the screen. (00:02:55)
The Patrol - S1-E8
Visible crew/equipment: When The Patrol is on its way, one of the boats tilts over and some soldiers are screaming they can't swim. At some point during that scene there is a wide shot with in the background the other side and a skylight. Right in the middle of that shot on the other side you can see a man walking away wearing a orange vest or jacket of some sorts. Might be the pyro-guy. (00:33:15)
Visible crew/equipment: When the trainees breach a door in the hostage scenario a camera and two crew members are visible in the shot. (00:22:00)
Visible crew/equipment: At the beginning when Sarah, John, Cameron and Derek are driving past the dead cows, the shadow of a camera can be seen on the inside of the passenger door that Sarah is sitting in.
