| 1 | Friends | When Joey is counting the floors of Ross' building (the first time) to find out where Hot Girl lives, he is wearing a black top with a zip. When he rushes over to Ross' building he walks down the hall and knocks on what he believes to be Hot Girl's door. In these shots he is wearing a dark purple button up shirt. As soon as Ross answers the door, Joey is wearing the black top with a zip again. |
| 2 | The Simpsons | This is a mistake for the introduction for the later episodes. When Homer screams, he turns round. You see this in a wide shot. There are no boxes to the right of the door in the garage. However, two just appear out of thin air when he runs through the garage. |
| 3 | Sex and the City | At the beginning of every show we see Carrie walking down the street and she gets splashed by the bus. First the bus has people on it, then in the next shot they all disappear. |
| 4 | 24 | When Nina's just been shot, and makes it to the barn to call CTU (around 7.10am), while she's speaking on the phone there's a cameraman visible on the right hand side (DVD). |
| 5 | Buffy The Vampire Slayer | When Giles is giving his speech about the Hellmouth the books Buffy's holding keep changing from facing him to facing Buffy and back again. |

| 6 | Family Guy | Brian's newspaper's name changes from "Daily Informant" to "Daily Tim...". The newspaper also turns upside down between the two shots. |
| 7 | Scrubs | In the apartment there is a cardboard box on a table, with a strip of blue tape on it and a tube of paper sticking out of it. A few shots later another strip of blue tape suddenly appears on the box and the tube of paper has vanished. |
| 8 | Lost | When Hurley is at the new house, after his mom hurts her ankle, he reaches into the car window to get the cell phone. When he begins to dial, the cameraman's arm moving about is reflected in the back chrome of the side view mirror. |
| 9 | Smallville | Clark's hands on the hood of Jeremy's truck shift in different shots from spread wide to in front of him and close together. |
| 10 | The X-Files | When Scully goes to Mulder's room, thinking she has the marks, after he has checked her we see Scully's hair is behind her collar, but when the shot changes to her going to sit down, her hair is now hanging out. |
| 11 | Will & Grace | When Jack holds his glass of water, he changes from holding the stem of the glass, to the actual bulbous part of the glass between shots. |
| 12 | Battlestar Galactica (1978) | When the Warriors are playing cards, Starbuck places his cards on the table near the gold coins, saying "A perfect pyramid. Unless there's a better hand, the pot is mine". His right fingers tap on the table and his cigar is in the ashtray. When the shot cuts, his hand is suddenly up by his mouth, holding the cigar, and the cards and coins have vanished. |
| 13 | Stargate SG-1 | Carter and Cadet Haley enter the Stargate and we see them through the "puddle." When they come out on the other side, their positions are reversed. |
| 14 | Charmed | In the scene where Prue comes home and finds Andy inside the manor. There is a shot where if you look behind Andy's shoulder you can see half of the body of the director in the background in the observatory room sitting in a director's chair. |
| 15 | Futurama | When Fry and Bender are in the museum and locked in the "Villains" room, Bender breaks the two girders in the window, and then his arms falls off. As he puts his arms back in, you can see behind him that the girders are back in place in the window. |
| 16 | Angel | At the end of the opening credits, you can see Angel's reflection in the puddle on the floor. Since he is a vampire, this should not happen. |
| 17 | The O.C. | When Ryan is lighting Marissa's cigarette with his, at the start of the shot it's full, then the shot goes to Ryan and it's still full. But then the shot goes to Marissa and if you look at Ryan's cigarette you can see it is almost all gone. Then the shot goes to Ryan and the cigarette is full again. |
| 18 | Frasier | When Frasier and Niles are eating the steaks at the restaurant that Martin brought them to, in some shots you can see Frasier's baked potato, and in others from the same angle it is not visible. |
| 19 | Seinfeld | After Jerry, Elaine, George and Kramer walk out of the orange stairwell, there is a shot as they come out the door. A car drives past. The crew is reflected in the window. |
| 20 | Star Trek: The Next Generation | In the episode "where no-one has gone before" from series 1, near the start, just as Riker says "you have all the time you need", the engine consultant stops and turns around. As he does so, look above his head and you can see a microphone hanging down. The camera quickly adjusts so it's not visible any more. |
| 21 | King of the Hill | After Hank and Bobby return from La Grunta, Hank parks the car halfway into the garage. Yet when Peggy washes the blood from the deer off the front of the truck, the truck is now on the driveway and not near the garage. |
| 22 | Fawlty Towers | When Basil goes to fetch the branch to give the car a thrashing after it breaks down, the car manages to move onto the pavement all by itself. |
| 23 | V | When the fake Donovan is trying to rescue Julie, he is shot from behind. But in the next shot, the scorch mark is on his chest. |
| 24 | Red Dwarf | When Rimmer leaves for his exam, the table in the room only has an ashtray on it. Moments later, when Lister locks the door, a magazine, a can and a mug have appeared on the table. |
| 25 | Blackadder | During the introduction, the snake tries to get away from someone who is supposedly Blackadder. He gets the snake. It's head is above a black tile. There is then a close-up of the snake, Blackadder wags his finger at it. It is now above a white tile. |
| 26 | Star Wars: Clone Wars (2003) | Season 1 Episode 3: Chapter III. After the ARC Troopers have destroyed the Droid tank, the Captain waves the team on. In that shot, the red coloration of his helmet's chin is missing (it is white instead). |
| 27 | Dawson's Creek | When Pacey picks Joey up to carrying her out of the party, he puts her over his right shoulder. In the next shot, just before they go out the door, she is carried over his left shoulder. |
| 28 | M*A*S*H | When Radar and Burns leave the tent for morning parade, Radar's clipboard acquires a pencil. |
| 29 | Star Trek: Deep Space Nine | Aron Eisenberg's name is misspelled on-screen as Aron Eisenerg. |
| 30 | Stargate: Atlantis | When they first arrive on Teyla's planet, the DHD is close to the gate. When the wraith attack, the DHD is much further from the gate. |
