Continuity mistake: When Bones scans the pod you can see the skull in the pod has its jaw open. However the skull picture on the scanner has its jaw closed.

Continuity mistake: When Bones starts to explain about the Aztecs, Hodgins is holding a boot upright. Camera changes and the boot is now upside down. (00:06:40)

Continuity mistake: Fisher is putting a glove on his right hand when Dr Saroyan walks in. It cuts between them a few times, then we see him turning to the table to explain about the bones and neither of his hands are gloved. A short while later he turns to put on gloves - he regloves his right hand, it goes back to Dr Saroyan for a second, then back to him turning towards the table and both hands are now gloved. Continuity mistake: In Brennan's segment of the episode, the cake on Angela's fork disappears after Brennan points out that saying Booth's aura is "as clean as it's ever been" was sarcasm.

Visible crew/equipment: A potential editing oversight can be found as Booth and Brennan are entering the shooting range by watching the space under the rifle of the third shooter. The gas can be seen coming from the spray can to the shooter's right, not the barrel of the rifle. (00:19:05)

Visible crew/equipment: After Fuentes explains to Saroyan why he's not wearing the new lab coat, Fuentes tells Saroyan that he'll be coming to her wedding because they're family, and as the camera pulls back we can see a T-mark on the floor at his feet. (00:18:20)

Continuity mistake: Near the end we see Hodgins driving a Mini and turn down an industrial estate. However when the camera cuts to inside the car, out the back window you can see suburban streets. (00:36:20)

However Booth doesn't swipe his pass to allow him entry. Whenever anyone walks up without swiping the card, a siren goes off and the security guard stops them. Regardless of who they are, as demonstrated in several episodes. (00:06:00)

Continuity mistake: At the very start Booth has his hands by his side then on his hips instantly. (00:00:05)

Continuity mistake: When Bones is shading on the piece of paper at the start, it goes from wide light shading to narrow and dark. (00:06:30)
Visible crew/equipment: Just after Booth gets the codeword from Decker, we see a shot of him and a boom arm can be seen moving in the door's reflection behind Booth. (00:32:30)
Answer: Answering my own question here but, having contacted various people involved in the production of Bones including Kathy Reichs, Hart Hanson, Eric Milligan, and Deirdre Lovegood who portrayed Taffet onscreen I finally have the answer. No-one know how she did it due to her constantly working alone. And she never revealed this information before her death.
