Visible crew/equipment: After Phoebe tells Cole she's pregnant she leaves in the elevator, and when it cuts to Piper, Paige, and Leo we can see the crew's grey mat on the floor under Piper and Paige, then in two following shots (from the same angle) the red T-mark is visible on that mat, at Paige's feet. Continuity mistake: While at P3, Leo tells Phoebe that she can't turn into a Harpy just by getting scratched, and when Piper cautions Leo to use discretion Piper raises her left arm to Leo, but in the next shot it's Piper's right arm that's raised. (00:16:05)
Visible crew/equipment: When the Dark Priest finds the box containing the Lazarus Demon, just as the Seer says that she can always count on him to help, the shadow of the boom mic dips into view at the top of the screen. (00:09:05)
Visible crew/equipment: When Phoebe tells Piper that she's on a deadline and hangs up the phone, in the next shot as Piper says that she's going to Phoebe at the newspaper, the shadow of the boom pole/mic is visible on the wall behind Piper, at the top of the screen. (00:26:45)
Continuity mistake: After Phoebe finds the Scavenger Demon page in the Book of Shadows the camera dollies out, and when Piper asks what happened with Cole, the crew's taped black mat is visible on the bare floor with no fringed area rug, then when Cole shimmers into the attic, in the next shot of Piper and Phoebe we see the fringed area rug at the bottom of the screen. Visible crew/equipment: When Sykes is about to kill Susan at the ADA'S office, Leo hits Sykes and orbs out with Susan, and in the next shot as Sykes sits up we can see the green tape mark on the floor, under his leg. (00:22:30)
Visible crew/equipment: After Piper summons Grams and then Patty appears as well, when it cuts to Darryl and Cortez entering the manor, the blue and white T-marks taped to the crew's black mat are visible, at the bottom of the screen. (00:34:00)
Plot hole: When Yen lo enters the manor in the attic. Where did the water on the floor he comes through come from? Windows were closed, no visible leak, and the sister took precautions to make sure there was no water in the house. Also after he attacks and gets the dragon blade, he exits the house through the sink filled with water. Piper had already said to make sure there was no surfaces of water in the house and had Phoebe and Paige shut the water main off.
Suggested correction: There is a previous scene that shows a visible leak in the windows and an obvious puddle of water on the floor of the attic. This is the same puddle they later enter limbo through. As for the sink, Phoebe (in Paige's body) explains that she had been soaking potion pots and forgot to drain the sink. Turning off the valve wouldn't dissipate water that is already in the sink.
We're Off to See the Wizard - S4-E19
Other mistake: Leo tells Phoebe that fire throwing is an upper level demon power, but we know Phoebe had that power in her past life, Piper even reminds her of this earlier in the episode.
Continuity mistake: When Anling unmasks Yenlo, Yenlo's left hand is covering his wound. In the next shot, his hand is down by his side. (00:03:50)
Other mistake: After they talk to the Angel of Destiny for the first time, they send Leo to ask the Elders if he is legit. After Leo leaves they find out about Agent Jackman, and the following events unfold. After Jackman busts them Piper calls for Leo and tells him "It's OK, you can orb" and he appears asking her "What about Agent Jackman?" Except that Leo was gone before they found out about him and the girls hadn't told Leo yet that he even existed. (00:09:09 - 00:21:58)
Other mistake: When Phoebe is trying to summon Angela the spell doesn't work and Leo says that she needs the power of three. We've seen this spell done with just one sister in the past and it's never explained why they'd need to do it together this time.
Charmed and Dangerous - S4-E13
Character mistake: When Phoebe is describing to Cole the demon she saw in her premonition, she says on the good side of his face he had tribal markings, and Cole says, "Kinda like Belthazar," and Phoebe agrees. Belthazar has no markings like the ones on the demon's face.
Revealing mistake: When Phoebe and Paige use Cole as bait to lure Piper to the manor, Phoebe levitates and kicks two furies at once. When they show the front shot of her kicking these two furies, we can see Alyssa Milano's stunt double. (00:34:25)
Continuity mistake: When Devlin uses the Ring of Inspiration on his subordinate, his hand changes orientation from vertical to horizontal. (00:10:35)
Plot hole: Paige is able to use the enchantress' powers, but in "Pardon My Past", Phoebe can't use her past life's fire throwing power and Leo tells her it's because her past life misused it so she lost it. The enchantress misused her powers so Paige should not be able to use them.
Continuity mistake: When Phoebe tries talking to Emma about Belthazor, Emma's hands are folded in front of her, holding her sweater closed. In the following shot when Emma turns around, her arms are now down by her sides, with her sweater open. (00:31:10)
