By the angle of the coffin door the candle would have easily fallen to the ground. If you look carefully you can see a thin black cord attached to the candle.
Visible crew/equipment: When the Muslim armies approach Brendan Gleason's castle there is a side shot of the Muslims army coming to a halt. You can see the marker stake with yellow tape at the top (used during production as a mark). (01:10:10)

Suggested correction: I've watched this scene a number of times on YouTube and I'm certain that this "crew member" is an extra. He is not wearing a T-shirt and shorts, his clothing is correct for an extra. There are also other extras sat cross-legged a few feet from him.
Visible crew/equipment: About one hour and fifty minutes in, the two stars get into a fight and are rolling around the floor. In at least one scene, a cameraman, with a handheld Panavision camera, is quite visible in the middle of the shot.
Visible crew/equipment: When Jake is in the bedroom with his wife (where he pours ice water on himself to calm down) you can see the top of some crew member's head in the foreground right below the camera lens.
Visible crew/equipment: A stock car carrying Mel Tillis and Terry Bradshaw drove into a hotel swimming pool. If you watch the car as it drives into the pool, you'll see the jump ramp built at the end of the pool.
Visible crew/equipment: When Charlie is shining Willy's shoes near the end, you can see the reflection of a crewman in Willy's glasses when he glances to his right. (01:38:25)
Visible crew/equipment: John pushes his newly captured prisoner into the back of the van Jane is in. As he closes the van door you can see an extra arm behind John's back moving over to hold the doors closed.
Visible crew/equipment: Towards the end of the film when Billy runs at the top of the screen towards the exit of the jail, you can see the film crew at the bottom of the screen.
Visible crew/equipment: During the scene where the whole family is first driving into Beverly Hills, in the scene where they give the finger to say hello, the camera crew can be seen in the reflection of some of the cars.
