Continuity mistake: When Jodie hits the deer her vehicle spins out. As it is spinning the headlights are not projecting any kind of visible beam. After the car stops, and she is facing the deer she hit, the headlight is now projecting a very bright beam to illuminate the deer on the road.
Continuity mistake: During the party, Stass the assassin, dressed as a waiter, hides his gun underneath a stack of white napkins. When he gets ready to pull the gun out, it is now between the napkins.
Sophia Lopez - S1-E4
Continuity mistake: When Matt talks to the "fluffer", her lollipop disappears from her mouth in between shots. She then raises her arm and puts the lollipop back in her mouth. (00:20:35)
You Can't Miss The Bear - S1-E1
Continuity mistake: When Nancy goes into her guest room with Silas and Quinne in it, the things she are holding change position in between shots. (00:28:35)

Continuity mistake: In one shot Grissom puts a shoe on its side with a pencil. In the next shot the shoe is upright again.
Revealing mistake: In the opening scene during the game, one of the shots of the boy is reversed. You can see that the text "Tartans" on his jersey is mirror imaged. (00:00:25)
Visible crew/equipment: When Nina's just been shot, and makes it to the barn to call CTU (around 7.10am), while she's speaking on the phone there's a cameraman visible on the right hand side (DVD). (00:12:40)
Revealing mistake: In the last scene of the episode, Angel kicks the guy out of the glass building. This guy's a vampire, and you can see his reflection in the glass windows. Vampires aren't supposed to have reflections.
Beware Greeks Bearing Gifts - S1-E12
Visible crew/equipment: As the people of Troy are about to be attacked by the warriors in the horse, the camera pans up to the side of the wall where Deiphobus is talking with another soldier. You can see a crew member crouching down behind them wearing a white and grey shirt. (00:30:35)
Running to Stand Still - S1-E6
Visible crew/equipment: When Susan is following Paul in the car she borrowed, we can see the reflection of a crew member on the right hand side of the window. (00:18:15)
Visible crew/equipment: When Claire runs away from Bob after he calls her "Ms. Bennet", you can see Bob's shadow and another person's shadow on the ground. Since no one else was standing next to them, it has to be a crew member, most likely a camera man based on the odd shape of the shadow.
Continuity mistake: The red Harvard sweater Andie is wearing disappears between shots.
Suggested correction: Actually, it shows her taking it off and continuing to talk to Jack.
Continuity mistake: When Izzie is looking at the patient board, there are some lines on it which have been wiped out. But in the next scene we actually see Meredith walk to the board and wipe these exact same lines off (even though they were previously rubbed out...) (00:34:05)
Visible crew/equipment: When Jim-Bob delivers Betsy's mail she's washing her hair outside, and as she flips her wet hair back we can see the microphone under her halter top, while her arms are up. Then when she walks toward the cabin with Jim-Bob, the bulge of the mic transmitter is visible at the back of her shorts. (00:06:20)

Revealing mistake: When Tony is telling Gibbs & Ducky that Abby got a hit on the dead sailor's fingerprints, Gibbs takes the file from Tony; the camera shows the file, which appears to be a legitimate profile sheet, but in the comments section, is the script that Tony just read aloud. (00:07:36)
Indoor Fun With Sammy and Robby - S8-E19
Continuity mistake: After Sam and Robin are done playing darts, Sam puts his darts on the pool table. The shot changes and the darts have moved.
Revealing mistake: When Briscoe and Green investigate the auto yard with the stolen, blue SUV, Briscoe handcuffs the owner. When he cuffs him, he turns him, showing his back and his hands behind his back. You can see the actor isn't cuffed, just holding the handcuffs as he walks. (00:06:35)
Continuity mistake: During the stakeout of the apartment building, in the exterior shots of the Clyde Motorcycles shop, Danny is peeking out through a ripped paper opening within the letter "D" in Clyde. However, in the interior shots the black tape letters of the word Clyde differs. The paper is ripped between the two letters "D" and "E" and that space within the "D" is blocked with molding, making what is in the exterior shots quite impossible. (00:15:40)
The Crossing - S1-E2
Visible crew/equipment: After Hank Dale tells Steve Pinkus that he wants to know "How it feels to get away with murder," Hank throws a punch, and in the next wide shot we can see the second cameraman's arms holding an additional camera, presumably to film the close-up, at the right side of the screen. Continuity mistake: After the drive-by shooting at the Federal Building, when Owen and Sam escort the defector and his girlfriend into the sedan, the license plate number is "5GOI417" which soon changes to "5PCI294" when Sam drives backwards. Then it changes back to "5GOI417" when Owen and Sam leave the car, and yet it's "5PCI294" again when Owen points his gun at the camera.
