Continuity mistake: When Gwan-su grabs the interpreter by his robes, the ribbons used to tie the man's hat can be seen in Gwan-su's fist. The ribbons are positioned towards the interpreter. Two shots after this, the ribbons are suddenly pointed to the right. (00:52:22)
Soccer + Desi + Merchant + Titan - S4-E5
Other mistake: A spectator has a vuvuzela in his hand. This is a horn that emits a very loud monotone. It is common at soccer matches where he is. But what is he doing with it? He is looking through it as if it were a telescope. (00:32:05)
Revealing mistake: When Iris is sitting in bed recording the message for Barry, she is holding her iPhone so that the start button is on the left side, but the "video record" button appears on the right. Additionally, her right hand covers the speaker area of the phone fully, including the front facing camera. It is impossible for Iris to be recording the video live and reveals that the video footage was recorded ahead of time. The illusion that the video is being recorded by Iris live is further ruined by the fact that the room lighting is turned off in the scene, but the recording is well lit. (00:02:55)
Continuity mistake: When Luke snaps the broom into two pieces, it's not a clean break, leaving a few inches of bare wood in the bottom part. A few shots later when we see both parts again, it's a much cleaner break.
Opera House Murder Case: File 2 - S1-E22
Other mistake: At the beginning of the episode, the characters are gathered thinking back about part 1 of the case. Amazingly enough, the propmaster's armpit ate the A of his shirt. Now he's not a "BADMAN", he's a "BADMN." (00:02:05)
Continuity mistake: When Kate goes to see the LARPers at the park to track down the trick arrows, her cross-body leather strap changes direction repeatedly. At first, it hangs from Kate's right shoulder to her left hip, but when Kate asks about the cop in their group, the strap hangs from her left shoulder. Then the leather strap reverts to the original way, only to switch yet again after the fist bump. None of the shots are flipped. (00:20:10)
Revealing mistake: When K.I.T.T turbo boosts near the end when chasing the 4x4 you can see the camera strapped to the side of the car. (00:42:35)
Continuity mistake: When Matice is given the coordinates and writes them down, his handwriting changes a few shots later.
Continuity mistake: Kate arrives in front of the newspaper. Directly behind her a guy a bit on the chubby side is arriving as well. But he already was in the crowd shown before, looking at the dummy. (00:11:40)

Continuity mistake: After they start free falling from orbit, Sonic thrusts himself towards Mario to attack him, and Mario has his body tilted while upside down so that his head is leaning towards his left side. But in the next shot as Sonic draws near, Mario is flipped so he's leaning to his right side towards Sonic. (00:17:40)
True Colors - S1-E6
Continuity mistake: Walter and Paige begin dancing and there is a lady dancing in a green dress behind them. In the next frame, that same couple is dancing across the room and then a second later next to Walter and Paige again. Walter's left hand holding Paige's in a dancing position also changes from open to closed. (00:26:05)
Factual error: In the scene, Floki walks towards a small waterfall today known as Kvernufoss. Floki is supposed to be the only inhabitant if not the entire Iceland, at least the region, as we are shown no sign of civilisation whatsoever, and as historical record tell us about Iceland in the early 800s (as the events of Vikings seem to take place). But a well beaten footpath is visible, leading to the waterfall at the end of the canyon. That path is an oft-travelled path tourists follow nowadays to reach Kvernufoss, a well-known site in the area. A path like that in Floki's time would simply not have existed (there is nothing in that canyon but that waterfall, no reason to go there if not for sightseeing) and represents a (tiny) intrusion of our modern world in a show set in the middle age Iceland. Beyond the Farthest Star - S1-E1
Other mistake: When showing Scotty on the bridge, some of the frames show more of his body than others. In those frames, you can see that he lacks legs.
Continuity mistake: When Silver Surfer is requesting counsel, he has a single beam of energy emanating from his right shoulder. When Gamora agrees to his request, he suddenly has a beam of energy emanating from his neck as well.
Visible crew/equipment: At the end, when Aunt Vi and Delilah return home, just as Miles walks into the house, the green T-mark is visible on the floor.
Shelter from the Storm - S3-E18
Continuity mistake: When Winn and Brainy hug, Winn starts the hug with one arm high and one lower, towards Brainy's waist. The shot cuts and both his arms are up and in the centre of his body.
Beauty and the Obese: Part 1 - S1-E1
Continuity mistake: The stripper in blue and yellow has some men watching her and are very close to her stage/table. The men on the left have no drinks or anything else in front of them. A few moments later after she takes her top off, it shows the man in a bow tie and the guy in a beanie sitting there still, but suddenly they are at a small table with drinks. Also the people around them vanish and a dog appears. (00:01:54)
