Plot hole: The mob hitmen are about to enter the Sweeny family's house to kill them when the police van arrives to move them to a witness protection safe house. In the next scene, the bad guys abduct Catherine and try to force her to reveal where the Sweenies have been taken. Why? They could very easily have waited and simply followed the van.

Continuity: When Frank takes Catherine into the break room to meet his crew, two styrofoam coffee cups on the table keep moving closer together, then farther apart and back again throughout the scene.
Continuity: During the scene in the old man's apartment, a broken lamp can be seen already on the floor in the background - before the fight begins and before one of the thugs is shown throwing it there.
Continuity: Frank and Catherine drive up to the loading dock and park in front of a bright orange 18-wheeler that's being unloaded in the background. When they get out of the car in the next shot, the orange truck has instantly jumped to a spot much farther down the dock, at least 50 feet away.
