Bruce Forsyth (season 1, episode 12)

Mistake Audio problem: In the ballroom sketch when Janice is talking to Zoot, her mouth starts moving before we hear her even say anything.

Harvey Korman (season 1, episode 13)

Mistake Visible crew/equipment: Several times during "Love Ya To Death", you can see Jim Henson's head popping up from under Dr. Teeth.

Mistake Visible crew/equipment: During "Veterinarian's Hospital" when Dr. Rowlf says, "Day in and day out, it's the same 'ol routine!" you can see a puppeteer's head behind him.

Juliet Prowse (season 1, episode 22)

Mistake Visible crew/equipment: When the group of pigs, chickens and frogs are singing "Temptation," the head of Dave Goolez (the guy who does Gonzo) is visible under the second chicken on the right several times during the sketch.

Zero Mostel (season 2, episode 2)

Mistake Visible crew/equipment: When Zero Mostel is dispelling the monsters, you can see a puppeteer's head to his right for a brief moment before it disappears.

Mistake Visible crew/equipment: As Animal hits the cymbals, the top of a puppeteer's head pops up for a split second next to the drum.

Judy Collins (season 2, episode 5)

Mistake Visible crew/equipment: While Link Hogthrob is singing, "I Talk to the Trees", you can see a puppeteer's head briefly during the song.

Mistake Visible crew/equipment: Right after J.P. Gross says, "What for? No money in hog calling!", you can see a puppeteer's head to his left.

Mark Hamill (season 4, episode 16)

Mistake Revealing: When Luke Skywalker is introducing his 'cousin' Mark Hamill at the end, you can tell it's a split screen by the fact that Luke looks right past him, even once he's on stage.

Diana Ross (season 4, episode 23)

Mistake Continuity: When Diana Ross finishes her first song, the bird has both his feet on the ground; in the next shot, the bird has one leg raised, without enough time passing to do so.
