Deliberate mistake: Every time the commandos are seen in the early episodes of this season (The Freshman and Fear, Itself) they wear ski masks to protect their identities. Starting in The Initiative, they stop doing this. The reason is that the audience needs to be able to identify the different commando characters, but it's still a discontinuity.

Continuity mistake: When the peasant woman gives Jonathan the crucifix in the carriage, the chain falls over the side of his palm. When he opens his hand to look at it, the chain runs between his fingers. (00:10:10)

Revealing mistake: On the DVD commentary, the director says that he only had a handful of the experiment cubes in Dining Hall B and used cardboard cutouts for all the others. Revealing mistake: As the team gets off the subway and heads for the door into the Hive, the Red Queen's POV shows a nifty infrared display in the lower left corner that marks their locations - it obviously matches up to the ones on screen. Continuity mistake: When Indy meets Hitler, there is a shot over Indy's shoulder that shows his lips are slightly parted (relaxed). Cut to Indy's face and he closes his mouth tightly.
