Continuity mistake: When Anakin, Obi-wan and Palpatine are running through the hall and get stuck in the ray shields, Anakin is facing forward. When it cuts he is suddenly facing Palpatine. (00:17:10)

Continuity mistake: When Padmé is brushing her hair on the balcony, her position changes between the second and the third shot, seen, for example, from the pattern on the floor. Her hairbrush and her arms change position between the first three shots too. (00:28:15)

Continuity mistake: After discussing whether Anakin is the chosen one or not, the ship Yoda, Obi-Wan and Windu are flying on goes in for landing. When it goes down there is no trace of clones on the ground where it is about to land. When it cuts several clones awaits them. Also two huge vehicles suddenly appears on the ground. (00:39:20)

Continuity mistake: When Count Dooku drops the platform on the unconscious Obi-Wan, Obi-Wan is not lying on the same part of the floor as in the previous shot of him, seen, for example, from the pattern on the floor. Later, when Anakin walks over to him, his position is different again, and the platform has moved as well. He is also lying under a different part of the platform. (00:12:45 - 00:14:30)

Continuity mistake: At the beginning of the scene where Obi-Wan, Yoda and Bail Organa discuss the future of Anakin and Padmé's children, Obi-Wan's hand is positioned against his chin. The position of his hand, as well as his left arm, changes between shots. (02:03:50)

Continuity mistake: When Grievous throws his staff on the window making it crack, the pattern on the cracks change between the shots. (00:19:50)

Continuity mistake: When Anakin is on top of the elevator before jumping back inside of it, the glove is missing from his mechanical arm. After he jumps inside, he is wearing it again. (00:10:30)

Continuity mistake: Just before Anakin and Obi-Wan throw each other through the air, in the first shot from above, they are duelling on the floor. In the next shot from the other side, their positions are different, seen from the pattern on the floor. (01:47:25)

Continuity mistake: When Padmé is talking to Anakin on Mustafar, her right hand keeps changing position between shots.

Continuity mistake: When Anakin is talking to Yoda about his premonition, the shadows and the intensity of the sunlight shining through the windows keeps changing, depending on the camera angle.

Audio problem: After Palpatine reveals to Anakin that he's a Sith Lord, he says, "But you're not sure of their intentions, are you?" When he stops talking, his mouth is still moving. (01:02:25)

Continuity mistake: When Anakin tells Obi-Wan, "I was arrogant, and I apologize." his hair is combed a certain way. As the conversation continues, his hair changes. (00:48:40)

Continuity mistake: In the speech hall when Yoda fights Darth Sidious, when Yoda falls down grabbing onto a panel, he holds on while sliding down, leaving scrape marks with his nails. When it cuts you can see the scrape marks have changed. (01:49:25)

Continuity mistake: When Obi-Wan and Yoda arrive at the Jedi temple to find all the slain Jedi, there is a wide shot showing Obi-Wan walking several feet ahead of Yoda. Obi-Wan then raises his hands and is about to remove his hood when it cuts and now Yoda is suddenly right next to Obi-Wan. (01:30:40)

Continuity mistake: When the Jedi Council discuss about Grievous being on Utapau and Yoda says "Act on this we must" Windu is sitting with his hands down. When it cuts he has his right hand to his face. (00:46:40)

Trivia: General Grievous is permanently coughing when he talks. The cause is revealed in the last episode of the animated Emmy-winning TV series "Clone Wars" (albeit technically no longer canon). In that episode, Grievous kidnaps Palpatine and before leaving, is attacked by Mace Windu, who destroys part of his chest armour, making him cough for the first time.

Question: During the lightsaber duel on Mustafar, why does Anakin even bother to try to make the jump that he afterwards regretted? Couldn't he had just simply used the force to push the lava towards Obi-Wan, killing him, and then make the jump to safety?

Answer: Maybe, but he is overconfident and impulsive. He has also let rage blind him. Anakin is probably the better fighter of the two, but Obi-Wan kept his cool and analyzed as he fought while Anakin fought angrily and ultimately foolishly which left an opening for Obi-Wan to maim him. It's said in almost every episode of Star Wars not to let fear and anger cloud your judgement. Here are the results.
