Continuity: When Padmé is in her apartment and Anakin is in the Jedi Council chamber, they look out across the city toward each other. The position of the setting sun and the direction of its light across their locations is not consistent given what we are shown about their relative positions and the directions they are facing.

Please note: There are a few minor issues, such as Obi-Wan saying he hasn't used that name "since before you were born", among others, which can easily be attributed to character mistakes, ie. mis-remembering things, rather than "real" movie mistakes.
Deliberate "mistake": In the final stages of the Obi-Wan/Anakin battle, they are fighting while floating over lava on a piece of metal. Just before Obi-Wan jumps to the high ground, we get a close-up shot of Obi-wan (lightaber pointing left off-screen) and then Anakin, (lightsaber pointing right off-screen) In this shot, Anakin's right hand is uncovered/real, and his left is gloved/mechanical, when it should be the opposite.
Other: During the scene of Anakin's transformation into Darth Vader, there is a close up shot of his face just before his helmet is put in place. All of his face is shown as being badly burned with the exception of his eyelashes, which are still impossibly there. Subjecting one's face to any heat source hot enough to sear one's face will also burn away one's eyelashes as well.
Continuity: During the Sidiuos and Yoda battle, the Emperor throws down several pods at Yoda that pile up on the floor.The pile changes in the number and position of the pods between scenes. This is especially noticeable when Yoda's lightsaber is shown falling to the ground, almost no pods are on the floor verses many pods when Yoda falls.
Continuity: In the Palpatine/Yoda light-saber duel during the second time they lock sabers, Palpatine's blade is pointed down, then in closeup of his face, his blade is at an upward angle. Then when it cuts back to Yoda, his blade is pointed down again even though blades were constantly locked. This may only be noticeable in widescreen.
Audio problem: In the scene where Anakin turns to the dark side, Darth Sidious says, "You're fulfilling your destiny, Anakin." As he says this, his head is shown from behind and is in shadow, but the words don't match the motion of his jaw, and his profile shows that his mouth is moving after the words are heard.
Continuity: When Palpatine appoints Anakin to be on the Jedi Council, as the first shot ends, they are quite far away from the large window and the column next to it. In the next shot, they are much closer to them. In the following shots from behind Palpatine, the background changes completely; for example, the window is gone, and a metal object and a shelf have suddenly appeared behind Anakin. Then, when Palpatine says, "I'm depending on you" they are suddenly standing in the doorway.
Continuity: When Dooku uses the Force to throw Obi-Wan through the air, in the shot where Obi-Wan hits the ground and is knocked unconscious, he's not holding his lightsabre, nor does it land anywhere near him (watch in slow motion for confirmation). When Anakin walks over to him later, it is suddenly lying next to him.
