Continuity mistake: When Wanda goes to Archie's office, she sits down to talk to him about procedures and during the scene the books on his desk change position as the camera angle changes. (00:30:05)
Continuity mistake: After Wanda's visit to Archie's office she arrives at Otto's. When Otto stabs the mannequin with his sword, the position/depth of the sword, as well as the position of his undershirt when it lands on the sword, all differ in the following shots. (00:33:00)
Visible crew/equipment: When Jamie Lee Curtis is leaving John Cleese's office, she kisses him goodbye and calls him Uncle. After she has left, you can clearly see the cameraman move into view of the mirror. (00:33:27)

Continuity mistake: When Ken lets the Doberman out of the car it has a pink cloth on it that falls to the pavement. When it cuts to the dog about to grab the old ladies' dog the cloth is nowhere to be seen. (00:51:10)
Continuity mistake: When the Doberman grabs the dog there is nobody in the background behind the old lady. When it cuts to the Doberman running away there is suddenly a couple walking in the background. (00:51:15)
Continuity mistake: When Ken gets the Doberman to attack the witness, the Doberman jumps out of the back of the van with the witness' undergarment on his back which falls on the ground. The camera angle changes and the undergarment is not on the street anymore. (00:53:18)
Continuity mistake: When Ken lets the Doberman loose out of the back of the van, you see Ken looking through the rear window to watch the action. Continuity mistake: Right before the Doberman grabs one of the witness' dogs, all 3 dogs are near each other, but when the angle changes 2 of the dogs are behind the witness and the dog that the Doberman grabs is in front of her. Continuity mistake: During the funeral for the witness' dog, you see her standing there with a friend standing directly behind her with her arm on her for emotional support. The camera angle changes and the friend is now standing along side of her. (00:53:43)
Continuity mistake: When Otto goes to see Wanda after the dog attack, the scars on Ken's face changes from when Otto gets to the apartment to when he leaves. (00:54:10)
Continuity mistake: When Wanda goes to meet Archie at the flat, she takes a few steps back from the doorway when Archie opens the door. The camera angle changes and she is now in the doorway. (00:57:04)
Continuity mistake: When Archie dances with Wanda at the flat, they stop and you see Wanda put her hands around Archie's neck. The camera angle changes and her hands are now on his shoulders. The camera angle changes again and her hands are back around his neck. Happens about 3 times during the entire scene. There is even a time when only one hand is on his shoulder. (00:57:44)
Continuity mistake: When Wanda is lying on top of Archie and Otto surprises them, Archie screams and his hands are down by his waist. When it cuts his arms are suddenly waving above his head. (00:58:35)
Revealing mistake: When Otto holds Archie upside-down, out the window, the rigid outline of the support is quite visible under Archie's pants' material. (00:59:55)
Continuity mistake: When Otto is hanging Archie upside down outside the window, as Archie apologises, you can see Otto's thumb visible as he is holding Archie's legs. When it cuts to a close-up of Otto Archie's trousers are now covering Otto's thumb. (01:00:10)
Answer: The Deluxe Edition that was supposed to come out on 8/1 has been delayed to 10/10 due to MGM changing their distributer.
