Visible crew/equipment: When the SUV is struck by the large truck and is pushed along while upside down, a camera mount can be spotted (helps if you use slow motion) and also, a tow truck can be seen actually pulling the SUV along the street.

Continuity: After the gas explosion in the school, Angela reveals that Alice is infected. The woman cop asks "When were you gonna tell us that?" and the next shot shows Alice with burns/blood on her upper left arm. In the next shot as Alice goes to sit down, the severity and position of the burns/blood changes.
Revealing: Nearly at the end, the scientists find Alice's body at the waterfall, one of the men lifts up the cloth to take a look at her face. Her face is badly burned on one side and she is pronounced dead. However, take a look at the left side of her neck, you can see her either gulp or take in a quick breath of air.
Revealing: At the beginning, when one of the black trucks has an accident, whatever is making the dramatic sparks is still running when that part of bodywork is off the ground.
Continuity: Watch the left side of the mouth piece in Alice's mouth when she is in the tank at the end. In the shot up close to her left there is a plastic support going up, in the next shot of her right the support is pointing down, and after the scientist says "can you hear me?" and the shot is close to her, the support isn't even there.
Continuity: At the end of the film when Alice gets her memory back, she grabs a pen and points it very close to a man's eye, then the camera changes shot still looking at Alice and the man but the pen is much further away from the eye than before.
Continuity: Toward the end, Major Cain storms aboard the helicopter, shouting "Why haven't we taken off, yet?" The shot switches to LJ (Mike Epps)in one of the pilot's seats. As he says, "Because I normally drive Cadillacs" he punches out at Cain with his left hand. In the reaction shot, however, as Cain flies backward, the punching hand is LJ's right hand.
Revealing: In the early aerial shots of Raccoon City, look to the left of the screen and you can see the CN Tower, a very recognizable landmark that is definitely in Toronto, not Raccoon City. This is due to the fact that film was shot in Canada.
Factual error: When any character's automatic pistol runs out of bullets, the slide doesn't lock open. All the weapons shown are of a type which should.
Continuity: When Nemesis blows up the STARS sniper on the roof of the theater, the size of his rocket launcher changes drastically.
Continuity: In the scene where the Girl is being chased by the zombies (just before we meet Carlos) the keypad for the door is right next to it. When she tries to enter the code, it is suddenly next to the small grey box.
Continuity: Nemesis has an ammunition counter for his Vulcan cannon. When Nemesis is shooting at Peyton on the bridge, his counter stops at 3997 rounds. Almost immediately he begins shooting at Jill, but now the counter reads 3688.
Continuity: Near the end, at the chopper crash site, the Umbrella man lifts the corner of the sheet covering Alice's body. From the high zoomed out shot, the guard is facing the waterfall, while the next shot he is facing the body.
Continuity: When Alice and the group are going to raid the Umbrella guards and steal the helicopter, one of the men looks through binoculars with night vision. He looks at the helicopter, then looks to the right of the helicopter, yet in the next scene a second later, his head is in the same position as it was when he looked at the helicopter, before he looked to the right.
Continuity: A zombie hits its head on a tombstone, which falls over. In the next shot, it's upright again.
Continuity: In the DVD, at the climax, just prior to when Alice is forced to fight Nemesis, she has a conversation with Thomas Kretschmann (the dude with the leather coat). During the conversation, one shot shows Alice with blood trickling down her neck and some bruises on her face after she previously had no injuries. Moments later, her face is back to normal. Later, during the fight with Nemesis, you see that that is where she sustains her injuries.
Continuity: When Jill Valentine and Angie are running from the dogs after turning on the gas, Jill kicks the left side of the double doors open. However, when she turns around to see if the match ignited the gas, she has the right door open. When Jill Valentine realizes Alice light a cigarette she starts running, again out of the right side of the door. (Looking at the doors from the kitchen to the cafeteria).
Plot hole: In the school cafeteria, Jill flips on six of the stove gas feeds. As she walks out, she tosses a match that fizzles, meaning that the gas hasn't reached the exit hallway. Seconds later, Alice manages to light the gas by flicking a cigarette at the doors at the far end of the hall just as the dogs break the glass. There's no way that the gas could have filled all that space in that amount of time. Matches burn hotter than cigarettes, so it's not that the gas simply didn't ignite before.
Factual error: The sniper on the roof who is casually picking off zombies hand loads his rifle after every shot. The rifle he is using is the AWP Magnum which has a spring loaded 10 round clip and a breach that cannot be hand-fed ammunition.
Deliberate "mistake": When Alice first fights the Nemesis, she attempts to jump over a fence and make an escape. When she lands on the police car, Nemesis fires his RPG at her. If you look closely, the rocket passes magically right through a chain link fence without touching it at all.
