Continuity mistake: In the opening scene of Level 10 (Sanchez' fortress), Sanchez tosses a stone after his lieutenant Pedro, which lands in front of the barracks under the water tower. Later on, in Sanchez' tutorial (Level 14), however, Sanchez finds the same stone - he recognizes it - right in front of his fortress' gate.
Continuity mistake: After the tanker in Game Over explodes, when you turn to look at Captain Price for the first time, there are only two people on the road in front of you. However, when you look back a second later, Gaz has suddenly appeared right in front of you.
Continuity mistake: In every single cut scene, even if your weapon is different, or if you have a different color set for the weapon, the weapons the characters are holding will become the original ones they were holding in the beginning of the game. The colors they are have absolutely nothing to do with you or the story picking them.
Continuity mistake: In the opening cutscene, when the mechanical bird, Mathew, shows a projection video display, it is in full color. But any other time during actual game play and cut scenes, the projection from Mathew is a grainy black and white.
Continuity mistake: During the Ornstein and Smough boss fight if you kill Smough first you see a cutscene of him falling dead onto his back, Ornstein then places his hand on Smough and absorbs his power. When this happens you can see that Smough has been flipped over onto his front and Ornstein is actually resting his hand on his back - note Smough's armour.
Continuity mistake: The game begins and ends in the same room, with the sleeping princess Zelda. There is a difference though: in the beginning she is on a bed sided by 2 braziers, which are gone in the ending - that's because they are an addition for the NES version. The 'in-game' version of the room was extended with columns all the way, and added those braziers. The ending loads the original Famicom Disk System version of the room, without them.
Continuity mistake: The family tree Kay and Griffin retrieve from the armory expedition is already visible in the hall of before the mission.
Continuity mistake: In the level Ambushed, after Diavolo leaves the room where Bond is with the two guards, they both have a gun pointed at Bond. But after Bond says "You first", the game shifts to actual gameplay, and the guy behind Bond no longer has a gun.
Continuity mistake: In the intro of the game, Archie Wister talks to a young prison officer. The radio on the breast pocket of the officer changes size and details between shots, together with other details of the uniform like a nametag.
Continuity mistake: Some of the missions - unintentionally - let Tanner switch cars if the player is quick enough (by stopping at red lights and swiftly jumping into another stationary vehicle, for example) and leave the default vehicle behind. Still, in the following CG cutscene, it's always the default/generic car that can be seen.
Continuity mistake: When Kratos is sent back in time by the Sisters of Fate to the point when he was fighting Ares, you can see that Kratos from the past has the scar on his stomach that was caused by Zeus much later in the series.
Continuity mistake: In the remake, in the cutscene after Jill fights Nemesis outside the clock tower, Jill will have the grenade launcher equipped, even if she didn't have it equipped before the fight.
Continuity mistake: In the finale of the KOF94 section, Rugal Bernstein's cybernetic eye switches from right to left between cinematic drawn cutscene and in-world ones.
Continuity mistake: Owen's tombstone is in the graveyard from the start of the game, preceding his death by a few days.
Continuity mistake: Claude obtains his default outfit after escaping from the police convoy. However, during the bank heist, prior to getting shot and arrested, Claude has these clothes already on.
Continuity mistake: The first time you arrive at Ecruteak City, go to the Burned Tower. After speaking with Eusine, you can see that Silver (your rival) is in the middle of the tower, waiting for you. However, instead of speaking with him, leave the tower, and continue the game until you reach Olivine City. When you arrive, Silver will come out from a gym, speak with you and then go away. However, if you return to the Burned Tower, you can see that Silver is still there, which means he was at two places at once.
Continuity mistake: In the flashback of Jade and her husband having an argument because of his infidelity, the painting behind Stephen has a broken glass before Jade throws the object that damages it.
Continuity mistake: Athena's sprite in the game has large buddhist-style praying beds (6 visible under her top's collar). In the artwork during the ending, the praying beads are much thinner (7 visible just in her 3/4 view).
Continuity mistake: If you travel to a planet other than Earth, then replay the second mission on Earth, your character says they need to obtain a warp drive, while they are actually flying through a warp.
Continuity mistake: Episode 3, Citizen Brown: Marty smashes the DeLorean window trying to get the tyre iron in the window. All of the glass that smashed suddenly disappears.
