Corrected entry: Even after La Forge wanders out of sick bay, the display above his bed continues to register his vital signs.
Correction: Not a mistake. As seen several other times in the series, the Enterprise's computer is capable of reading the life signs of it's crewmembers anywhere on the ship. That LaForge's signs appear on the medical display, is simply a matter of output chosen previously by the doctor.

Corrected entry: In the episode "Skin of Evil", right after Commander Riker gets pulled into the sludge type black substance watch the left side of the screen. When the away team looks into it after Riker's face comes out of the sludge you will see something plop into the goo, that was Geordi's phaser falling out of his holster when he leaned over the pool of goo.
Correction: Not a mistake, as it is possible to lose things like this in real life. The mistake is, however, that the phaser reappears later, which is already listed on this site.

Corrected entry: Riker says he reviewed the logs of the encounter with Q, where the alien kidnaps Troi, Picard, Tasha, and Data. But later he's unaware that Troi is a member of the crew.
Correction: Riker not only says that he reviewed the logs; he is also seen actively doing so on orders from the Captain. But more importantly, he never states that he was unaware that Troi was on board, he merely pauses and stares at her when they meet. This is because Riker and Troi have a romantic history together, so it is natural for him to have an emotional reaction when they meet again. To say that Riker was unaware of Troi's presence is an assumption, nothing more.

Correction: Riker suggests this three times, and all three times his lips move just fine. It is hard to see the first time because of the distance between him and the camera, but his mouth is moving as it should.
