When the SIDRAT materializes, in the shot facing the door as The Doctor pokes his head out to exit, the camera turns toward the right and there's a reflection of the moving camera and the camera operator walking behind the camera, at the right side of the screen. Revealing mistake: Episode 6, while in the barn Von Weich asks Moor (played by David Troughton, son of Patrick, who plays The Doctor) to retrieve the monocle in his pocket. Then, when Von Weich continues with his plan, in his closeups we can see a vertical piece of mesh tape over the actor's right eye. (00:13:05)

Visible crew/equipment: (Episode 5) When Vicki is in the jungle she happens upon the TARDIS and bangs on the door begging to be let in. In the next shot we can see the boom pole/mic hovering in the background, then a hand appears which seems to guide the actress to head in that direction. (00:07:45)

Visible crew/equipment: (Episode 4) After Ian shudders from the apparition he continues to follow The Doctor down the hall, and the camera casts a shadow on Ian. Then, when The Doctor finds the stairs leading to the laboratory he tells Ian he must see what's on the table. Just as Ian states his displeasure at the thought, the boom mic (and its shadow) pops up into view at the bottom of the screen, in front of Ian. (00:05:15)

Deliberate mistake: During all the animated parts of "The Power of the Daleks" in numerous shots various illustrated character's such as The Doctor, Polly, and Ben have their buttons, pockets, patches, etc, flipped to the opposite side, sometimes even within the very same shot.

Continuity mistake: After Skagra demands the book from Professor Chronotis, as Skagra's sphere floats toward Chronotis he's wearing his eyeglasses, but then the Professor's eyeglasses vanish and reappear while his hands are raised.

Continuity mistake: During the animated portions of Shada's missing scenes, in numerous shots an illustrated character's clothing (buttons, scarf, etc) become flipped, sometimes even within the same shot. Example: when The Doctor goes to the physics lab and meets Clare Keightley, note her buttons and The Doctor's scarf.

Visible crew/equipment: After Chris exits the TARDIS with the medical kit, Romana places the collar around the Professor's neck. When Romana tells Chris that Chronotis, "Should be able to think with it," in the next closeup of Chris the moving boom mic is visible at the top of the screen, before he kneels down.

Visible crew/equipment: (Ep. 2) When The Doctor, Jo, and Dr. Tyler are taken into Omega's palace, in the first shot of the multiple passageways (right before Jo says that it looks like Aladdin's cave) the curved chalk actor's mark is visible on the floor, then we see it again in three following shots. Note that when they're all escorted further inside, just as Dr. Tyler comments about the matter/anti-matter not making sense, Jon Pertwee looks down, presumably to make sure he hits his mark.

Continuity mistake: (End of Ep. 3 and start of Ep. 4) When The Third Doctor fights the dark side of Omega's mind, the rings that The Doctor wears on the pinky fingers of both hands switch to his ring fingers, then back again to his pinky fingers, between shots. Then as Omega chokes The Doctor, the ring on his right pinky vanishes between shots.

The Three Doctors - S10-E1

Revealing mistake: (Part 4) After Omega takes off his mask and looks in the mirror, when he sees that his physical body doesn't exist he puts his mask back on and screams, but when he shouts, "If I exist only by my will, then my will is to destroy," Omega's mask pops up off his face, and we can see the actor's blackened face as he shouts the last bit of his dialogue. Visible crew/equipment: (Part 3) After Omega has revealed himself to The Third Doctor, Jo, and Dr. Tyler, when he begins to speak privately to The Doctor the camera casts a shadow on Omega's body. Then after Omega turns around the boom mic dips into view at the top of the screen. And when Omega starts walking forward, the shadow of equipment (presumably the boom and mic) is moving on the far wall, near the lower left of the screen. (00:03:10)
