Trivia: Regina Baff was difficult on set. Jane O'Brien who played Terrible Tommy revealed the match was supposed to end in a draw but Regina complained to the producers who forced director Robert Fowler to change it. She also shouted at Fowler more than once during filming.

Spider-Man 2 (2004)
Deliberate mistake: Perhaps to avoid getting an R rating or parental complaints notice how in the operating room massacre none of the doctors appear to bleed nor do Doc Ock's tentacles have blood on them.
Suggested correction: The problem is in both versions of the film, basically all of the deaths either happen off-camera, or are things like people being shocked to death or killed by being tossed around and presumably having their backs/necks broken. Most of the deaths would be bloodless or near-bloodless anyways, and the camera simply doesn't linger on the bodies enough to show any blood. This is also why the tentacles aren't really bloody. Sure, it was probably ratings-related, but it isn't technically a mistake.

Batman: Arkham Knight
Continuity mistake: This is in the same universe as the other Arkham games yet Poison Ivy's gone from being green in Arkham Asylum and Arkham City to being just regular looking in this game.

Species (1995)
Other mistake: It's impossible for Sil to wear the conductor's clothes. She's tall and slim around maybe 5'9" while the conductor looks to be 5'5"or so and a little chubby.
Suggested correction: It's not "impossible" in the slightest. Both women were roughly the same height. Natasha Henstridge is about 5'10" and Esther Williams was usually listed around 5'9" or 5'10" before she passed away. And the weight difference also makes sense, as the clothes look baggy and ill-fitting on Sil. (Ex. The jacket is very loose and hanging around her shoulders and arms, and the shirt isn't nearly as form-fitting as it was on the conductor.) It's pretty much perfectly portrayed in the film.

Revealing mistake: When Fred and Ethel come with parts of their apartment's ceiling on them look closely at Ethel. The piece on her left shoulder falls off revealing what looks like a velcro pad to keep it in place.

Injustice For All (2) - S1-E19
Other mistake: Unless he escaped prison, it makes no sense for Copperhead to be in the police van with the other Secret Society members. You can see him sitting there across from Solomon Grundy and The Shade.

Insurgent (2015)
Continuity mistake: Divergent and Insurgent take place after one another it makes no sense that Jeanine goes from wearing dark blue to a lighter blue as she does here.

The Karate Kid Part II (1986)
Question: Did Kristy Swanson and Randee Heller decline doing the sequel or were they too busy with other movies?
Answer: If referring to the blonde in the first movie, that was Elisabeth Shue, not Kristy Swanson.
Okay my mistake but did they decline returning or what?
Answer: To add to the previous answer, Elisabeth Shue went back to Harvard to continue her studies which is why she doesn't appear in the sequel.
Answer: Ali was originally only supposed to have a very small role in Part 2. Elizabeth Shue declined to reprise the role because she was studying at Harvard at the time. I can't find any explanation online as to why Randee Heller was not in Part 2 and was barely in Part 3.
Answer: Like all studio executives they think mixing things up by bringing in new characters into different settings would keep the franchise fresh. Most of the sequels from the 1980's were given minor or major cast changes.
