I don't think there's enough detail in the wide shot to say it's a different woman or not. She's seems to be carrying the same clipboard in the same way. Plus, in the next shot, the first woman should still be seen somewhere in the wide shot off to the left, and she's not.
Again. In the wide the woman's hair is up. You can even see a pin in it. It's not in the first shot. Also, she would not necessarily need to be seen in the wide shot. The first shot has them in the elevator, then there's a hallway, before they cross the doorway into the lobby. She would have already walked past the desk (notice the lamp) by the time they were a few inches out of the elevator. By the time they walked to the doorway she could easily be out of frame.
Impossible. The very short distance she walks takes her almost 2 seconds, and then she's off camera for maybe a second. The distance to the hall to be out of the shot is almost twice the distance she was just seen walking.
Depth perception. She's walking directly in front of the desk. She's not in the same spot as the second lady (in front of the lobby door). She walks across the screen before Max's feet leaves the elevator. The distance between the left side of the desk and off screen is fairly short and plenty of time for her to cross before they exit the elevator. Third time...different hairstyle. I'm pretty sure the second lady is even wearing a different shirt underneath her jacket too.

Continuity mistake: When Greta is packing her belongings in a box, she grabs a glass ashtray from behind Fletcher. She puts it in the box, then decides not to pack it and lays it behind the box. When Fletcher says 'Greta, don't leave!' the ashtray is back in its original position. (00:44:10)

Continuity mistake: Just after Fletcher steals the yellow airfreshener from the car yard, he has his arms by his side. When the shot switches back to him, he has his arm on the counter. Continuity mistake: Just before Fletcher steers his flight of stairs under the wing of the plane, there is a shot taken from underneath the wing of the plane and you can tell by looking at the runway markings that the plane is suddenly barely moving, it appears to be moving about 5 miles per hour compared to about 30 miles per hour in previous shots. Continuity mistake: There are a couple of instances in the movie where the models do not correspond with the actual sets. When Klytus arrives at the Hawkmen city he lands near the top of the city. Yet in that very hall there is a pit with a deep void underneath. Such a room could only exist at the bottom of the city. Factual error: Sharaz Jek's monitor that displays the tunnels and his androids is inaccurate. The white blobs are, as we learn later, supposed to represent his androids. He tells the Doctor that the green area is held by the army, yet on the screen the green area is populated by androids, so they could not have had control.

Revealing mistake: When Stotz cuts a hole in his ship's door, you can see that the inside of it is made of white polystyrene. They painted the outside of it but obviously forgot about the inside. Also listen to the sound it makes when Stotz punches the hole through - it sounds exactly like polystyrene.

Factual error: When the Doctor is trying to land the ship, from the video screen it appears that they are moving towards the planet way too fast to make such a safe and quick landing. Also, the ship doesn't seem to have even penetrated the planet's atmosphere yet it lands seconds later.

Continuity mistake: At the beginning of the movie when the scout is talking with Longstreet, when the scout holds up his finger and says "you didn't know they were on the move out there", a long stream of troops is seen marching to the right. Continuity mistake: The dead bodies of the SWAT team move around on the escalator between shots. The best example of this is after the last SWAT guy dies, there is a body or two near the end of the escalator. They disappear or at least move much further away after McClane drops his gun on the escalator. And all this happens while the escalator is not moving. (00:45:15 - 00:48:00)
