Continuity mistake: Spoiler! At the end of the episode, after Elektra decapitates Alexandra, the blood pattern on her changes changes when she turns around and asks "any questions?" Most noticeably more blood on her forehead and two streaks down her chin that suddenly appear. (00:49:10)
Continuity mistake: Episode 6.4, Libra Slayer: In the opening scene, in the hospital, the man leans over the dying patient to hear what he has to say. His cheek is almost touching the patient's lips. Then the angle changes and their faces are suddenly quite far apart.
Glory Days - S8-E3
Continuity mistake: Towards the end of the episode, during the final shootout, Murdoch ambushes an armed bad guy in the attic and jumps through the wooden wall with him. As they fall down, Murdoch's trademark hat spectacularly flies off his head, but in the next overhead cut - showing him and the bad guy lying on the ground - the hat is on his head again.
Factual error: Gay pride flags are visible in a window. The 6 color flags shown were introduced in 1979 (though an 8 color flag preceded it in 1978). (00:30:44)
Continuity mistake: When Vaughn's assistant puts down the newspaper in front of him, it switches position to the left between shots. (00:38:20)
Visible crew/equipment: After Raines shares details with Kellett about Semien's crew, when it cuts to Semien waking everyone up, just as he announces his contact with Belgian Federal Police, there's a visible T-mark taped to the floor behind Andre's cot. (00:24:25)

Continuity mistake: When Denton looks up Alex Campbell's vehicle registration, it says "AP08 00G" on the computer screen. When she goes to where the vehicle is, the registration is "AP08 0DG."
Continuity mistake: When Jimmy gets to Chuck's burned house and sees the coroner truck with his brother's corpse getting away after the beginning of the episode, a fireman is going to pass behind him, but when Jimmy turns to see again the burned house, the fireman has suddenly disappeared when it turns into a long shot. The Woman Who Never Died - S1-E3
Continuity mistake: When the culprit backs off from the door because of detective Katsumoto and drags Emily with him, she has her left shoulder covered with hair, a little hair, then no hair at all. Even more noticeably, when Higgins tells him that he does not have to do this etc, he is pulling her with him using his hand encumbered with the trinkets bag, but at the cut his hand is empty and directly on her bare shoulder. Continuity mistake: When Jerome get stabbed in the neck, in the first shot the blood comes from his mouth and goes towards his eye. In the next shot, there's no blood trail going to the eyes, but instead to his cheeks. (00:34:50)
Continuity mistake: Kai tosses the rock fragment next to the dagger, but in the following shot the distance between the two has increased dramatically. (00:31:50)
Revealing mistake: The scene is set in Boston, MA, USA, but at a murder location, a European-style circular power outlet is visible through the glass doors at the bottom of the concrete wall. (00:01:20)
Visible crew/equipment: When Jimmy Smits and Dave Navarro are talking to the shopkeeper, a microphone wire can be plainly seen under Navarro's wife-beater. (00:14:25)
The Thing About Heroes - S5-E2
Other mistake: Officer Burgess kicks in a door with a lock and deadbolt and does no damage to the door frame. (00:46:21)
Continuity mistake: When Claudette, Danni, Julien and Wagenbach are at the overturned churro cart crime scene, Claudette is seen pulling a pair of white gloves from her pocket. The gloves then disappear from her hands in a wide shot of her talking to Julien about the crime scene, but reappear in the next shot. (00:16:45)
Episode #1.1 - S1-E1
Continuity mistake: Episode 1 - When the commissioner kisses his wife at the beginning of the first episode, he is holding the holstered gun in his left hand and his ID in the right hand. That's in the first shot, because his daughter enters the room bringing coffee, and now the two items swapped hand. (00:05:10)

Visible crew/equipment: When Booth walks over to Laura and Barney at the speakeasy, the moving boom mic casts a shadow on the wood pillar behind Laura, at the top left side of the screen.
Prodigal Son - S5-E3
Visible crew/equipment: After Agent Hobbs reads the text that Zack sent to Clayton, Jubal tells ADIC Reynolds they need to evacuate Chapman, and in the next shot facing Reynolds there's an orange T-mark taped to the floor behind him. (00:19:35)
