Terminal - S3-E13

Revealing mistake: To conceal their unauthorized entry into the secret lab, Cally very carefully closes and locks the sliding door. But when she turns back to speak with Tarrant, the "secured" door rolls halfway back open by itself. No, it's not Servalan's troops sneaking in to catch them: there's no sign of anyone there. Besides, anyone unlocking the supposedly-locked door would have alerted them. A case of malfunctioning BBC scenery, plain and simple. (00:43:45)

Gold - S4-E10

Revealing mistake: Avon and Soolin hide from the processing plant guards in an alcove, but Avon's black-booted foot is sticking out in plain sight. Fortunately, the guards don't seem to notice and walk on by anyway. (00:18:00)

Season 4 generally

Revealing mistake: The muzzles of the the crew's guns are usually empty, but when they need to actually fire them, you can see a black insert sticking out which is the little pyro charge.

The Web - S1-E5

Revealing mistake: The double-bladed axe one of the Decimas uses to attack the station windows is wobbling as he strikes with it. Small wonder he's not getting far at breaking in, with a weapon made of rubber. (00:22:30)

Stardrive - S4-E4

Trivia: The box-like entrances to the underground lair proved problematic for the Space Rat actors. When several of them were supposed to spill out on cue, the box walls and their heavy helmets prevented them from hearing the director, who repeatedly screamed "Action!" to no avail. A lone voice from inside the box eventually shouted back, "Can't hear you!"
