Factual error: The premise of the film is swapping the faces of the Hero and Villain. We can see the whole face of Sean Archer is intact in a water tank, which includes his eyelids and lips. When Castor Troy wakes up and confronts the doctor, despite his face being missing, he still retains eyelids and lips, which isn't possible as they would be missing. This should result in Castor having exposed eyes and teeth. (00:46:47 - 00:48:08)

Visible crew/equipment: When Burt shoots at the Shrieker with the high-powered rifle, a frame of two before the Shrieker explodes, a wire is visible at the tip of its beak, holding it up. (01:16:45)

Deliberate mistake: Bowser has several teeth that appear and disappear throughout the movie. The best example of this is when Mario is trying to grab the Power Star. We see Bowser snarl at Mario before he picks him up. There is a gap between his two front teeth. However, when the scene cuts from Mario's horrified face to Bowser's face, there are now extra teeth inside that gap.
