Other mistake: After the first guard at the chemical plant is sniped, the second guard goes to check on him and is surprised by the sudden appearance of an attacking helicopter. Helicopters are extremely loud, and flying as low as it was depicted would also vibrate the ground and the building. There's no way it would have been able to sneak up on the guard like that.

Factual error: The movie makes a mention of a CIA black site in the South Asia Sea. There is no such sea. Between the North Pacific Ocean and the Indian Ocean there are many "seas", the largest being the South China Sea. But there are also others such as the Andaman, Java, Banda, Timor, Celebes, and Sulu seas. There is no South Asia Sea.
Factual error: A clandestine team of men and women is being transported to near the coast of Vlavidostok to parachute down and capture nuclear detonators. They are shown being transported in a US Air Force C-130 Hercules. This is a no-no. The CIA does use the Lockheed L-100-30, the civilian version of the Hercules, but would never have Air Force markings for a clandestine operation, especially inside Soviet airspace.
Trivia: Sylvester Stallone originally exited this film in 2017 over creative differences with producer Avi Lerner. Arnold Schwarzenegger exited as well in solidarity with Stallone. The film ended up being shelved for several years until Stallone returned, albeit in a much smaller and final role. Schwarzenegger, as well as other key actors from the franchise, ultimately did not return.
Trivia: The timer on the bomb is a rotary kitchen oven timer. Amazon sells various types identical to it. I've got one myself. Not necessarily a mistake, because the bomb seems to be somewhat improvised, but still amused me.
