Continuity: At one point about three-quarters of the way through the movie, when Lowell Bergman (Al Pacino) and Don Hewitt (Philip Baker Hall) are talking, watch in the background as a certain lady walks past after Don says something. When we cut back to him he says something else, but the same woman in the background appears and walks exactly in the same direction, just like in the previous cut.

Continuity: Halfway through the film, Al Pacino phones his assistant who is sleeping. She turns on the light and picks up the phone. In the background you can see a tall building with all the lights on. When the director cuts back to her the building is not there.
Factual error: When Weigand and Bergman are meeting for the first time in the Seelbach Hotel, Bergman asks Weigand if he's always lived in Louisville. The shot shows some small buildings on the outskirts of downtown Louisville, implying that what is being shown is downtown Louisville. In actuality, the main business district of downtown is on the other side of the hotel.
Continuity: Check the glass of water on the table in front of the little girl. When they shoot from afar, you can see the level of the water is lower than when she actually picks it up to drink it.
