Continuity mistake: James lit the cigarette he gave Amanda, and she was shown taking one puff. Amanda tells part of the Empty Man story and the ashes on the tip of her cigarette are the same when she takes a second puff. Character mistake: Jean rolls, kneads, and flattens the dough a few times, repeating the steps to give the image of still preparing the dough. After the dough was flattened, it was well-shaped (round, higher, and more compact) then flat again. The way she was shaping the dough suggests she was making biscuits, but biscuit dough should not be handled that much (or the biscuits would be flat and not flaky). (00:18:00 - 00:19:00)
Factual error: Real-life Martina Franca is a town in Puglia, at the "heel of the Italian boot." The town shown in the film is the famous San Gimignano in Tuscany. And when Holly "defects" in order to go to the Hill of Tara, her departure point, as seen on Foaly's observation monitor, is the real-life location of San Gimignano. (00:35:50)
Revealing mistake: The candles on the long dinner table are numerous and clustered, but linear in front views of people seated at the table. Based on how the candles are disorderly arranged, it would not be possible to show a couple or few candles spaced apart in front of one person. (00:05:35)
Continuity mistake: Early in the film when Jake gets in his car to head off to pick up his son, his seatbelt is on although there is no sight of him putting it on, then, as he pulls away the seatbelt is off.
Continuity mistake: The belongings that the half-brothers packed into the cargo area of the vehicle are inconsistent many times during their trip. Sometimes, none of the belongings can be seen through the windows. (00:28:09 - 00:57:45)
Continuity mistake: Maggie was holding two bottles (clear containing a green liquid) in her right hand and set them on the table together with one at an angle behind the other. She moved her hand away and when the camera shifted to a wide view, the bottles were side-by-side (horizontal to the screen) with more space between them. Also, both bottles were almost full, but had less in them when Maggie handed one to Grace at the airport. (Maggie may have had a sip out of hers, but not Grace's; both looked unopened). Continuity mistake: The shape, amount, length and width of the white down "Dream Alliance's" face varied, as did the height of his white "boots", indicating that the same foal/horse was not used throughout the movie. The white on the foal's and adult Dream Alliance's face and lower legs looked painted on at times. Before Dream's first race, black tape was used on his lower legs, apparently as an attempt to make a different horse's higher white boots less or not at all noticeable. (00:32:42 - 00:35:51)
Continuity mistake: Take notice of the desk lamp as Charlie and Al are talking in Charlie's new office. As the camera switches back and forth between their face to face conversation, the green glass cover of the light keeps changing position too. This happens even after Al leaves. This was probably done to keep the light out of the camera during filming. (00:13:50)
Continuity mistake: Views of Michelle's car wreck were inconsistent. When the car veered to the right and was headed toward the tree, the driver's view showed most of the car was to the left of the tree trunk. On impact, it looked like the right side headlight hit the middle of the tree trunk. Michelle got out and a rear view showed the tree trunk roughly lined up with the steering wheel (i.e, more than half of the car was to the right of the trunk). After impact, both headlights were shining in front of the tree.
Factual error: Rich told Jamie, "There was a car accident. Guy came in. I cut when I should've stitched or stitched when I should've cut. I don't even know. His family sued in civil court, and I lost everything." A doctor working at a hospital would have been required to carry malpractice insurance, which would have protected him from "losing everything." Also, it would be difficult to prove that a car accident victim who died during surgery was a victim of malpractice; most cases are settled out of court. (00:38:30)
Revealing mistake: Near the beginning of the movie, there does not appear to be a curtain (or window) on the left side of the screen (e.g, when Samantha is sitting on the couch), but there is near the end when Chloe enters the now-empty room. Continuity mistake: At the "Wave Hair Salon", Ms. Gordon has a curler/roller at the top of her forehead. In some shots, it is straight, but in other shots it is angled. A little later, Christine goes over to Ms. Gordon and starts rolling down the curler to the top of her forehead as if for the first time. (00:08:04 - 00:08:47)
Continuity mistake: The location of the "no parking" sign near the front of Dick's Restaurant changes. Initially, the sign is near the front door. When the logger drops off the envelope for Debbie (from Jim), there's no sign by the door, but there is one past the building. Deliberate mistake: Zhang crushed and smeared several long red-hot peppers (cayenne?) in his hands to use as a makeshift weapon against thugs when rescuing Mei Mei. After Zhang smeared and pushed peppers into one thug's eyes, it is unlikely he would have been able to retain as much of the crushed peppers in his palms as he did. Also, holding the peppers in one or both of his palms would have interfered with grasping the second thug and redirecting his gun (without sliding off). (00:08:00 - 00:08:50)
Continuity mistake: There are only two missing pieces to complete the jigsaw puzzle, and the perimeter/frame is intact. After the pastor turns the puzzle around to face Eden, there are four pieces missing, three of which are perimeter pieces. (00:04:38)
Factual error: The movie is set in the 80s (you can see tell for instance by the driving license of the old woman, which uses the 80s template and has a 1985 date), but the police plays the suicide tape on a Panasonic PV-4614, which is a late 90s model of VHR. Same goes for the Sony video camera used to tape the murder, definitely from the 90s. (00:19:00)
Continuity mistake: The protagonist receives her gift from Molly Shannon. She unwraps it and puts the discarded wrapping on the counter. When we see it again at the end of the scene, the wrapping is much less torn apart than the first time around (or at least has been arranged neatly). (00:09:45)
Other mistake: When Lomito was holding the little green ball with the smiley face on it, her flashlight was shining on her fingers/fingernails - all of which were long and manicured. The length of her fake fingernails - even if they did not exceed the department's policy on maximum length, such as one-fourth inch past the fingertip - probably were long enough to interfere with her ability to quickly put her finger on her pistol's trigger and shoot; the length interfered with her ability to perform her duties.
