Revealing mistake: After Howard’s death, Tony and Rhodey are trying to get back into Stark International. A guard is giving them a hard time and he is wearing some sort of shades and head piece. When it cuts to a close up of his face as he talks to the people inside, on the left side of his glasses near where it meets his hair, there is a texture glitch in the animation. The glass in one spot is flickering around like TV static. (00:07:10)
Continuity mistake: When Geralt fights the Striga, he ends up lying on his back with the Striga on top of him. To escape, he uses his magic to "punch" through the floor. The first attempt fails, its only effect is to blow away some dead leaves. At the second attempt a few seconds after, leaves are back on the floor.
The Incomparable Connie Walker - S1-E18
Continuity mistake: "The Incomparable Connie Walker": When Dillon visits Mayor Walker's house, Walker's red carnation is on his left lapel. But one shot of him is, for some reason, reversed, causing the carnation to switch itself to his right lapel for a moment.
Independent Dependents - S1-E7
Plot hole: When Helen goes into the air ducts, she simply removes the grates. They would not be loose like that, they would be firmly attached. When the team enters the basement they go through an ordinary door by picking an ordinary lock. A very high security installation like Axe Industries would not have such an insecure entry, even into the basement.
Continuity mistake: Gar enters a room and leaves the door open. Throughout the whole mirror scene the door is also open both when the camera looks at him and when the camera points at the mirror. However, when he is about to leave the room he needs to open the door as it has been closed inbetween us looking at Gar in the mirror and looking at him normally. (00:17:50 - 00:18:35)

Other mistake: In the ending shot of the tournament bracket showing the potential future opponents, the two saints' names are written the wrong way, as "DORAGON" and "ANDOROMEDA", instead of Dragon and Andromeda. (00:22:00)

Visible crew/equipment: After Bud wakes up all alone in Bligh's cabin, he soon realises all the doors are locked. Then, just as Bud rushes over to the stern window (particularly the second time), a seated crew member is visible at the bottom right corner of the screen; we can see the movement of a leg and two hands (so, it's not a mannequin).
Carmelita Is One of Us - S3-E25
Continuity mistake: In full shots of Kelly holding the baby, it is entirely covered by the pink blanket. But in close-ups, the blanket is pulled back to completely expose the baby's head. (00:17:00)
Visible crew/equipment: As the old Indian sets fire to burn his dead son, he walks towards Cain and on the left you see a fire extinguisher. (00:10:04)
The Monsters Are on Maple Street - S1-E32
Continuity mistake: During the disturbance, the crystal punch bowl shatters, spilling glass and punch all over the table. Subsequent shots show the punch bowl still intact with no punch spillage.
Factual error: 1-1 A couple of spelling errors. A "community center" in Ceylon, a British colony, using the American instead of the British spelling. And Columbo listed as the capital of Ceylon; it's Colombo.
What If... Captain Carter Were the First Avenger? - S1-E1
Deliberate mistake: When Peggy and Steve are studying the map where the 107th Army division is held captive, the names on the map are nonsensical names that do not even follow German spelling or standards for place names ("Farfignugen," "Dädtenorth," and "Rosskjhberg," for instance). Most of them are also reused, appearing several times on the same map. This is most likely done deliberately so as to avoid any resemblance to real places, but still looks out of place and illogical. Deliberate mistake: Season 2, episode 6 "Identity Crisis": When Bizarro starts decaying into the parody form of Superman, his costume goes with it; the blue fades, and even the 'S' insignia changes. Strange that superhero garments change right alongside their wearers when they suffer from an unstable genetic effect. Were the clothes cloned right onto his body, instead of sewn?
The Blessing of Many Fractures - S1-E8
Continuity mistake: During the fight with Zhou Cheng, Cheng does a forward flip and kicks Danny, making him stumble backwards. In the shot of Cheng lying on the ground, Danny is holding his stomach. In the shot immediately afterwards, he's holding his chest.
Let Sleeping Zords Lie - S1-E7
Continuity mistake: Kendall is wearing black heeled boots when she is fighting the aliens, switches to black sneakers when she lands after being thrown and has the boots again when she stands back up.
Bug Off/The Crawl Space - S9-E5
Continuity mistake: In Moving Away, Tommy tells the other Rugrats how they first met when Angelica lied about the nasty things they did. However in the Crawl Space, there is a flashback of Tommy, Chuckie, Phil and Lil playing in the playpen when they were younger. Chuckie is scared of the Jack-in-a-box in that flashback, so the Rugrats could not have met for the first time back then.
