Continuity mistake: When the class is in Arnold's esophagus, Tim says something, then it cuts back to Ms. Frizzle. Arnold is sitting in the back seat of the bus.
Plot hole: When Dominique is on TV, she says she got the shoes from Al's store. Then the Amish guy comes out to confirm she broke her leg from the shoe he made. Al says yes. She got the shoes way before Al knew he had no more shoes and before they even hired the Amish guy. How can she break her leg from his shoe if he wasn't hired and making them yet?
Suggested correction: Al was passing the blame.

Revealing mistake: During the fight between Garak and Worf near the end of the episode, a Jefferies tube door is knocked out of its frame to reveal the end of the set - a dark room with a wooden scaffold and various other pieces of equipment (slow-mo helpful but not required). (00:36:00)
Vint and the Kids Move In - S1-E1
Continuity mistake: When Vint moves in, he stays in the basement. Mama, Fran, and Buzz all have a "bedroom." Sonya moves into Fran's studio. Then when Naomi thinks she's pregnant, they argue over where Vint and Naomi will move to instead of the basement. Fran says she won't give up her room, Buzz says his room is in the attic. Sonya says her room is microscopic. Thelma has the biggest room. Then on Mama's Birthday, she gets Buzz to help clean out of the attic, which obviously wasn't his room as he stated. Then, of course, after Buzz, Sonya, and Fran are all gone there's only room for Thelma and Bubba upstairs.
Continuity mistake: Tony's hair length alternates between a short buzz cut and longer in different shots in the scene where Sil, Paulie and Chris visit him after his mother dies.
The Friendly Physician - S2-E29
Plot hole: The scientist yells for Igor to capture the escaping Castaways, to which Igor responds "capture them yourself" in Ginger's voice. The scientist appears surprised to find out Igor is not Igor, but how can this be? He had to be the one to pull the lever to switch everyone's personalities and bodies around, so he should have already known.
Star-Crossed Lovers and Other Strangers - S1-E16
Visible crew/equipment: When Rory asks Lorelai to call Emily to get her out of Friday night dinner so she can celebrate her and Dean's 3-month anniversary, a boom mic drops into the shot as Lorelai stands to get the phone. (00:07:05)
Plot hole: They're in the Clam complaining that they can't drink when they make the revelation that Brian can because he's technically 56. But Quagmire should be able to drink, as in a much earlier episode, he's revealed to be over 60.
Suggested correction: Quagmire considers the rest of them to be his best friends. He isn't drinking because they can't drink. He is complaining because since his friends can't drink - he won't drink, so he is still pissed.
It's still a plot hole considering Quagmire's age. I think the writers messed up big time.
You make a good point, but it's awkward to be that one person who is drinking while surrounded by people who can't. So, I understand the correction that was submitted above. Because his friends are not drinking, he now feels that he can't.
Character mistake: Throughout season 3, whenever someone visits Henry in prison, the word "VISITOR" on their visitor tags is misspelled as "VISTOR."
Family Ties - S1-E22
Visible crew/equipment: After Rygel's transport pod returns to Moya, as the docking bay door slides open, in the shot facing everyone while they have their weapons drawn we can see the bare floor. However, when Aeryn checks Crais for hidden weapons and then as D'Argo approaches him, there's a blue crash mat on the floor. Then it vanishes again.
Factual error: A thief has stolen 75 million dollars. He runs out with two large duffel bags. All US bills weigh a single gram. This means that $1,000,000 in $100 bills - the largest bills in circulation - weigh around 10 kilograms, or a little over 22 pounds. $75 million would be 1,650 pounds. No way this could be carried in two duffel bags.
Other mistake: During the Pokérap, when the Poliwrath name is said, the Pokémon shown is actually Poliwag. Also, when the Graveler name is said, the Pokémon shown is actually Geodude.
Character mistake: When Neal tells Rebecca about an Indian Idol and describes the idol to her, she tells him it's the Hindu Idol of Seeta, the goddess of wealth. Actually the idol is of Goddess Laxmi.
Girl Trouble / School Dance - S3-E18
Continuity mistake: Besides the fact that when Helga is sharpening her pencil, the desk arrangement changes, one of the students visible when she looks at Arnold is...Helga.
Continuity mistake: When General Edel and Martin get in their car at the gas station, the fuel pump nozzle is still in the fuel port of the car. They drive off and the fuel pump nozzle has miraculously removed itself from the car.
Factual error: After escaping from Danzig, Grzegorz and Konrad flee to Warsaw, then to the eastern, Soviet-occupied part of Poland. They then make their way on foot right across occupied Poland and Germany to join British forces at Dunkirk, a distance of over 1,000 miles. Not a likely journey on foot at any time, let alone across hostile territory in the middle of a war.
Factual error: In Oregon and New Jersey, state law requires that a gas station attendant pump the gas. Romero pulls into an Oregon gas station and encounters a nosey person. They are both pumping their own gas.
Visible crew/equipment: When Chester and Cassie are in The Cabin, Chester is looking through the picnic basket while Callie holds on the Indian's hand and talks to him. In the scene behind Callie is an unidentified "cowboy" who appears twice and then disappears when the scene cuts to Chester the 2nd time. (00:47:00)
Revealing mistake: When Sgt. Troy throws the grenade at the Gestapo agents barricaded behind the table, it is thrown wide right, yet the pyrotechnic explosion is dead center of the table barricade.
